摘要:
目的 观察哌唑嗪治疗血液透析伴肾性高血压对患者体液和神经指标的影响.方法 选择接受治疗的74例血液透析伴肾性高血压患者为观察对象,予以回顾性研究,根据治疗方法分为观察组(哌唑嗪治疗)与对照组(缬沙坦治疗),各37例.比较两组体液指标和神经指标.结果 治疗前,两组患者各观察指标均无显著差异(P>0.05).治疗后,两组患者血管紧张素、血清肾素、醛固酮三项体液指标水平明显降低,且观察组水平明显低于对照组,差异均有显著性(P<0.05);两组患者窦性心搏RR间期(SDANN)、RR间期标准差(SDNN)、RR间期差值>50 ms个数的百分比(PNN50)、RR间期的均方根(RMSSD)四项神经指标均明显升高,且观察组明显高于对照组,差异均有显著性(P<0.05).结论 哌唑嗪治疗血液透析伴肾性高血压,能够对血管紧张素系统产生抑制作用,有效调节交感以及副交感神经功能,可以作为血液透析伴肾性高血压治疗的首选方法.%Objective To observe the effect of prazosin on neurological and humoral indexes during hemodialysis in patients with renal hypertension. Methods 74 patients with renal hypertension treated with hemodialysis in this hospital were selected as subjects for observation,a retrospective study had been conducted, and they were divided into observation group (treated with prazosin) and control group (treated with Valsartan) by random allocation method, and 37 cases in each group. The indexes of neurology and body fluid were compared between these two groups. Results There was no significant difference between these two groups (P >0.05) before treatment. After treatment, three humoral indexes as angiotensin, serum renin and aldosterone in these two groups were significantly decreased, and these levels in observation group were significantly lower than those of control group, and the difference was significant (P < 0.05 ). Two groups of patients with sinus cardiac RR interval(SDANN),RR interval standard deviation (SDNN),the difference in percentage of RR interval > 50 ms number (PNN50),root mean square RR interval (RMSSD) four neuropsychological indicators were significantly increased, and these in observation group were significantly higher than those of control group, and the difference was significant (P < 0.05). Conclusion The application of prazosin in treatment of patients with renal hypertension during hemodialysis, the inhabition of renal angiotensin system (RAS) can effectively regulate the sympathetic and parasympathetic nerve function, hence it can be used as the preferred method for treatment of patients with renal hypertension undergoing hemodialysis.
