    • 摘要: 推进乡村卫生服务一体化管理是深化医药卫生体制改革的重要内容之一,也是强化村卫生室服务能力和职能作用的现实需要。伴随城乡居民医疗保险制度的实施、基本公共卫生服务均等化项目工作的推进,将村卫生室纳入乡镇卫生院一体化管理,全面推进乡村卫生服务一体化改革,是满足农村居民医疗卫生需求的必要举措。结合河北宁晋县全面推进乡村卫生服务一体化改革的实践经验,对广大农村地区的医疗卫生工作有一定的借鉴意义。

    • 摘要: 目的 了解乡村全科医生职业倦怠现状,并对其影响因素进行分析.方法 2020年5月,采用多阶层随机抽样方法,选取山东省西南3个县区共2 805名乡村全科医生作为研究对象.采用自制量表调查全科医生个人基本情况,采用职业倦怠量表-人类服务工作者版(MBI-HSS)调查其职业倦怠情况.采用百分比、频率等统计指标描述乡村全科医生工作倦怠的现状,通过Pearson x2检验和二元logistic回归模型,分析乡村全科医生职业倦怠的影响因素.结果 回收有效问卷2 272份,有效率为81.0%.在情绪耗竭、去人格化倾向和个人成就感降低维度中,处于中高度职业倦怠的乡村全科医生中分别有1 251名(55.1%)、772名(34.0%)和2 075名(91.3%).在二元logistic回归模型中发现,影响乡村全科医生情绪耗竭维度的危险因素包括平均每周工作时长≥60 h(OR=1.341,95%CI:1.057~1.702)、平均每月夜班次数≥20次(OR=1.434,95%CI: 1.182~1.739)、平均每天接诊时长≥13h(OR=1.294,95%CI: 1.055~1.589)、接诊患者数11~20例(OR=1.317,95%CI: 1.075~1.614)、不参与兼职(OR=1.583,95%CI: 1.201~2.087)、存在失眠症状(OR=2.638,95%CI: 2.225~3.128)、在工作中感到沮丧(OR=3.170,95%CI: 2.661~3.776);影响乡村全科医生去人格化倾向维度的危险因素包括参与公共卫生服务(OR=1.485,95%CI: 1.155~1.910)、在婚(OR=2.273,95%CI: 1.198~4.313)、不参与兼职(OR=1.677,95%CI: 1.276~2.203)、存在失眠症状(OR=1.487,95%CI:1.211~1.827)、在工作中感到沮丧(OR=1.642,95%CI: 1.339~2.015);影响乡村全科医生个人成就感降低维度的危险因素包括平均每周工作时长≥60 h(OR=2.089,95%CI: 1.454~3.002)、平均每天接诊时长≥9 h(OR=2.495,95%CI: 1.748~3.561)、存在失眠症状(OR=2.061,95%CI: 1.519~2.796)、在工作中感到沮丧(OR=1.894,95%CI: 1.383~2.593);其中保护因素包括月收入≥3 000元(OR=0.589,95%CI: 0.353~0.982)、平均每月夜班次数10~19次(OR=0.581,95%CI: 0.411~0.820),均P<0.05.结论 山东省西南地区乡村全科医生整体工作倦怠情况严重,尤其是去人格化倾向和个人成就感降低维度.建议提高乡村全科医生工资待遇,减少乡村全科医生工作强度,优化乡村全科医生工作时间,同时关注他们的身心健康的问题,从而缓解乡村全科医生职业倦怠的现状.

    • 摘要: 目的 了解山东省西南地区乡村全科医生离职倾向及其影响因素,为维持我国基层医疗队伍的稳定性提供合理建议.方法 2020年5月,利用多阶段随机抽样的方法对山东省西南地区2805名乡村全科医生进行问卷调查.采用Pearson x2检验和多因素二元logistic回归方法分析乡村全科医生离职倾向的影响因素.结果 共发放2805份问卷,回收问卷2693份,有效问卷2272份,有效率为84.4%.有1076名(47.4%)乡村全科医生存在中高离职倾向.二元logistic回归结果显示,兼职(OR=1.443,95%CI:1.105~1.884,P<0.01)、平均每月夜班次数≥20次(OR=1.340,95%CI:1.106~1.623,P<0.01)、接诊时长≥13 h(OR=1.358,95%CI:1.107~1.666,P<0.01)、存在失眠症状(OR=2.075,95%CI:1.755~2.454,P<0.01)、工作中感到沮丧(OR=2.987,95%CI:2.516~3.546,P<0.01)、有情感衰竭(OR=3.801,95%CI:3.188~4.533,P<0.01)、有去人格化倾向(OR=2.493,95%CI:2.086~2.981,P<0.01)的乡村全科医生更易产生离职倾向.结论 山东省西南地区乡村全科医生具有较高的离职倾向,兼职、平均每月夜班次数多、接诊时间长、失眠、感到沮丧、职业倦怠是离职倾向的影响因素.政府部门应加强对基层医疗服务的资金投入,提高乡村全科医生的工资待遇,降低工作负荷,增强乡村全科医生队伍的稳定性.

    • 摘要: 目的 了解安徽省乡村医生现状.方法 以分组随机问卷访谈的形式调查安徽省乡村医生基本信息、执业资格、技能培训、待遇收入及新农合政策下农村卫生室现状等.结果 安徽省村医老龄化问题逐步凸显,50岁以上村医占比28.79%;整体素质有待提升,研究生及以上学历占比仅1.61%;人才培训进展缓慢,阻碍乡村医生向执业(助理)医师转变,近3年中,47.82%村医未接受过培训;在新农合政策的实行下,49.76%农村卫生室统一配置的医疗设备不能满足当前农村医疗卫生环境的需要.结论 安徽省乡村卫生室设备有待改善;重视加强村医队伍建设;逐步提高村医的福利待遇;全面均衡城乡医疗资源配置.

    • 摘要: 近年来,我国农村地区落后的急救体系建设与急救服务需求之间的矛盾日益严峻.为了进一步提升急救质量和效率,国家大力推进农村地区胸痛中心、卒中中心、创伤中心、危重孕产妇救治中心、危重儿童和新生儿救治中心“五大中心”建设,以构建高效的农村急救服务网络.作者通过收集国家有关“五大中心”急救建设的政策文件以及相关地区“五大中心”建设的实践做法,借鉴国外急救体系和医疗救治中心发展的经验,从内外部环境、功能定位、资源整合、运行机制4个方面重点分析了“五大中心”建设创新急救体系发展的作用机制.针对农村地区“五大中心”建设与院前急救及院内急救的统筹整合、信息化建设、人员急救能力建设、利益分配等,提出了相关政策建议.

    • 摘要: Objective To survey the current status and training requirement of rural doctors in Hebei province.Methods A questionnaire survey was conducted among1 200 rural doctors selected from 11 cities of Hebei province by stratified sampling from October to December 2017.The self-designed questionnaire included general condition,job satisfaction,impact of medical reform policies,training needs of rural doctors.Results A total of 1 200 questionnaires were distributed and 1 170 valid ones were retrieved with a recovery rate of 97.5%.Among the 1 170 responders,there were 726 males(62.1%) and 444 females (37.9%);748 (63.9%) were aged more than 40 years;612 (52.3%) hold secondary school degree;826 (70.6%) had been working in the village for more than 10 years,and the overall job satisfaction rate was 30.8% (360/1 170).Among all responders,73.8% (864/1 170) worked in the standardized village clinics,and 72.0% (842/1 170) were contracted with local residents as family doctors;52.8% (618/1 170) responders believed that the reform policy improved the health care for villagers.In all responders,98% (1 147/1 170)thought they needed training,93.6% (1 095/1 170) had received training.The training contents they most needed werethe management of common diseases (904,77.3%),followed by medical humanities(650,55.6%)and first aid knowledge (492,42.1%).The influencing factors for training were lack of time,no energy (656,54.3%),lack of appropriate training forms (510,43.6%),economic reasons (476,40.3%) and so on.Conclusion The working status of rural doctors need to befurther improvedand the training need to be strengthened to increase their job satisfaction.%目的 了解河北省乡村医生工作现状及培训需求,为乡村卫生事业建设及乡村医生培训提供政策建议.方法 2017年10-12月,按照分层抽样的原则,抽取河北省11个地级市现任乡村医生1 200名,采用自行设计的问卷进行调查,内容包括乡村医生的一般情况、工作满意度、医改政策的影响、培训需求等.结果 共发放问卷1 200份,收回有效问卷1 170份,有效回收率为97.5%.1 170名接受调查问卷的乡村医生中,男性占62.1% (726/1 170),40岁以上者占63.9%(748/1 170),中专学历者占52.3%(612/1 170),从事乡村医生年限≥10年的占70.6%(826/1 170),对工作总体满意度为30.8%(360/1 170);被调查者回答所在卫生室为标准化村卫生室的占73.8%(864/1 170),已施行家庭签约的占72.0%(842/1 170),认为新医改政策改善村民看病难、看病贵的占52.8%(618/1 170);98%(1 147/1 170)乡村医生认为需要接受培训,93.6%(1 095/1 170)表示曾经接受过培训;对于上级部门组织的专业技能培训应为哪些方面内容的问题,根据被调查者的选择,依次排序为常见病、多发病(904人,占77.3%)、人文医学(650人,占55.6%)、急救知识(492人,占42.1%)等;关于乡村医生进行培训的影响因素,根据被调查者的选择,依次排序为时间、精力不足(656人,占54.3%)、缺乏适宜的培训形式(510人,占43.6%)、经济原因(476人,占40.3%)等.结论 各级政府部门应重视提高乡村医生的工作满意度,完善乡村医生培训体系,提高乡村医生业务水平,深入落实家庭医生签约服务,使乡村医生真正成为农村居民健康的“守门人”.

    • 摘要: 目的 了解山东省乡镇卫生院参与医疗共同体(以下简称医共体)的策略.方法 借鉴新制度主义理论、理性选择理论和社会网络理论探讨乡镇卫生院参与医共体建设的策略.采用问卷调查法于2018年8月对山东省13个地市264所乡镇卫生院的院长进行调查,调查内容为参与现状、动因、行为等.对各选项进行统计描述和双变量Pearson相关分析.结果 理论分析结果显示,乡镇卫生院行为受制度环境和技术环境的双重约束:针对制度环境的合法化压力,采取"稳妥的顺从策略"参加县域医共体,同时采取对自身有效率的具体应对行为;面对市场竞争压力,部分卫生院采取"积极的探索策略",积极搜寻合作伙伴,同时为市场探索寻求制度合法性.实证分析结果显示,264所卫生院中,有231所(87.5%)参与医联体.231所参与医联体的乡镇卫生院中,199所(86.1%)是应政府要求参与,161所(69.7%)是为了自身发展;对方技术水平(83.5%,193所)和资源匹配性(77.9%,180所)是选择伙伴的主要原则;220所(95.2%)参与县域医共体,66所(28.6%)同时参与其他类型医联体.结论 乡镇卫生院县域医共体参与行为高度趋同,同时采取了多种联盟参与策略.%Objective To analyze the motives and tactics of township health centers ( THC) to participate in the countywide medical communities of Shandong province. Methods Such tools as the new institutionalism, rational choice and social network theories, as well as the constituent ratios and Pearson correlation coefficient of data were used in a survey in August 2018 with managers of 264 THCs in Shandong. It aimed to learn from the respondents their motives, tactics, and behaviors to participate in such communities. Results Theoretical analysis of the results found that their participation was bound by both institutional environment and technical environment. For example, compliance tactics, accompanied by efficiency, was adopted to cope with stress from institutional environment. Exploratory tactics, accompanied by legitimacy, was adopted to cope with stress from technical environment. The figures showed 87. 5% of 264 THCs investigated participated in such communities, and their main motives were in turn to follow government requests (86. 1% ) and to seek development opportunities (69. 7% ). The critical criteria of partner selection were in turn technological level (83. 5% ) and matching of resources (77. 9% ). Of 231 THCs which participated in the communities, 95. 2% joined in countywide medical communities, and 28. 6% participated in other forms of medical alliances. Conclusions THCs showed high organizational convergence in their participation, and adopted diversified strategies of participation in medical alliance.

    • 摘要: 目的 了解乡镇卫生院卫生技术人员工作满意度现状,探讨有效提高工作满意度策略.方法 基于生存、相互关系、成长发展需要理论,运用定量与定性相结合的方法进行研究.定量调查采用多阶段整群抽样方法,以课题组编制的乡镇卫生院人员调查表为测量工具,对45家乡镇卫生院803名卫生技术人员进行生存、相互关系、成长发展需要3个方面的调查,采用描述性分析、方差分析对定量资料进行分析;定性调查采用方便抽样方法进行个人深入访谈,并使用主题框架法对访谈内容进行分析.结果 乡镇卫生院卫生技术人员生存需要满意度得分最低,为2.59分;其次是成长发展需要,满意度得分为2.91分;相互关系需要得分最高,为3.56分.人员类型、性别、年龄、是否在编、有无行政职务、职称不同,其生存、相互关系、成长发展需要得分存在显著差异.结论 管理者应针对乡镇卫生院卫生技术人员的生存、相互关系、成长发展需要,通过完善薪酬分配制度、健全人员培训体系等激励手段,提高其工作满意度.%Objective To understand the job satisfaction of health technicians at township health centers and explore the strategies of improvement.Methods Based on the theory of existence,relatedness and growth needs,both quantitative and qualitative methods were used in the study.The quantitative investigation was carried out with the multi-stage cluster sampling method.The questionnaire for township health center personnel was compiled by the research group as a measuring tool for a survey of 803 health technicians from 45 township health centers regarding their existence,relatedness and growth needs in terms of existence,relatedness and growth needs.The data acquired were subject to descriptive analysis and variance analysis.The qualitative investigation was made by means of convenient sampling in personal indepth interviews.The thematic framework method was used to analyze the contents of the interviews.Results The satisfaction of existence needs of health technicians at township health centers scored the lowest(2.59),followed by that of growth needs(2.91),and the highest scoring was relatedness needs(3.56).There exist significant differences in the scoring of existence,relatedness and growth needs,among different personnel category,gender,age,staffing quota,administrative posts,and academic titles.Conclusions The authorities should emphasize the existence,relatedness and growth needs of health technicians at township health centers,and improve their job satisfaction by perfecting the salary distribution system and perfecting the personnel training system.
