摘要:
目的:探讨溃疡性结肠炎患者血清及结肠组织α1-抗胰蛋白酶水平与肺功能损害的相关性。方法选取溃疡性结肠炎患者124例为观察组,选取同期健康体检人员124例为对照组,均行血清及结肠组织α1-抗胰蛋白酶水平检测和肺功能检测。比较观察组不同病变范围、病情程度、病情分期、病程各检测指标的差异性,分析α1-抗胰蛋白酶与肺功能指标的相关性。结果观察组血清及结肠组织α1-抗胰蛋白酶水平均明显低于对照组。远端溃疡性结肠炎患者与广泛结肠炎患者血清及结肠组织α1-抗胰蛋白酶水平均明显低于溃疡性直肠炎患者。病程≥5年溃疡性结肠炎患者血清及结肠组织α1-抗胰蛋白酶水平均明显低于病程<5年溃疡性结肠炎患者。观察组肺活量、用力肺活量、第1秒用力肺活量、第1秒用力肺活量/用力肺活量、肺总量、一氧化碳弥散量、FEF25%、FEF50%、FEF75%、峰值流速、最大中期呼气流速均明显小于对照组,残气量、功能残气量均明显大于对照组。α1-抗胰蛋白酶与肺活量、用力肺活量、第1秒用力肺活量、第1秒用力肺活量/用力肺活量、肺总量、一氧化碳弥散量、FEF25%、FEF50%、FEF75%、峰值流速、最大中期呼气流速均呈明显正相关性,与残气量、功能残气量均呈明显负相关性。结论溃疡性结肠炎患者可伴有不同程度的肺功能损害,患者血清及结肠组织α1-抗胰蛋白酶水平明显下降,α1-抗胰蛋白酶与肺功能损害有一定的相关性。%Objective It is to explore the correlation ofα1 -antitrypsin in serum and colon tissue with lung function injury in patients with ulcerative colitis ( UC) .Methods 124 patients with UC were selected as observation group, and 124 healthy person at the same stage were selected as control group.Levels ofɑ1 -antitrypsin in serum and colon tissue and lung function were detected in both groups.The differences in every indexes among different lesions ranges, degrees, disease stages and courses were observed in observation group, and the correlations betweenα1 -antitrypsin and lung function were analyed.Re-sults The level ofα1 -antitrypsin in serum and colon tissue in observation group was significantly lower than that in control group.The levels in the patients with distal ulcerative colitis and extensive colitis were lower than that in ulcerative proctitis patients.The level in the ones with disease stage >5 years was lower than that with stage <5 years.The lung function inde-xes such as VC, FVC, FEV1 , FEV1/FVC, TLC, DLCO, FEF25%, FEF75%, PEF and MMEF were all lower, while RV, FRV were higher in observation group than those in control group.α1 -antitrypsin was positive related with VC, FVC, FEV1 , FEV1/FVC, TLC, DLCO, FEF25%, FEF75%, PEF and MMEF, and negative related with RV and FRV.Conclusion The patients with UC has lung function injury more or less, in which the levels ofα1 -antitrypsin in serum and colon tissue obvi-ously decreased, and there was some relationship betweenα1 -antitrypsin and lung function injury.
