    • 摘要: 目的:探讨数字化技术设计股前外侧皮瓣(Anterolateral thigh flap,ALTF)修复重建颌面部缺损的可行性及精确性。方法:纳入2019年6月-2020年6月笔者医院口腔颌面外科收治的15例颌面部缺损患者,采用彩色超声对股前外侧皮瓣穿支的位置进行定位,对下肢进行CT扫描获取CT参数,利用软件对股前外侧皮瓣进行设计,对皮瓣的体积进行计算并进行皮瓣的制备,对术中实际皮瓣的体积进行测量。术后随访3个月,观察皮瓣成活及供区愈合情况,对口腔功能及外形美观进行评分,比较术前超声定位的皮瓣穿支位置与术中实际穿支位置的符合率,术前设计皮瓣体积与术中实际皮瓣体积的差异。结果:15例皮瓣均成活,13例供区为一期愈合,2例供区经过1周的换药后愈合;患者的口腔闭合、言语功能、咀嚼功能、吞咽功能及外形美观评分分别为(8.752±0.70)分、(8.83±0.79)分、(8.90±0.44)分、(8.24±0.55)分、(8.99±0.72)分,术前超声定位的皮瓣穿支位置与术中实际穿支位置的符合率为86.67%;术前设计皮瓣体积与术中实际皮瓣体积比较,差异无统计学意义(P>0.05)。结论:数字化技术设计ALTF修复重建颌面部缺损具有较好的可行性及精确性。

    • 摘要: 目的 探讨游离腓骨肌皮骨瓣修复颌面部缺损效果.方法 随机抽选本院2019年1月~2020年5月接收的42例下颌骨及软组织缺损患者,所有患者均实施吻合血管的腓骨肌皮瓣游离移植对下颌复合组织缺损进行修复,分析修复结果.结果 42例患者腓骨肌皮瓣均成活,短时间切口愈合状态良好.经过X线检查提示:3~6个月能够很好愈合.开口均正常,咀嚼功能及语言功能均正常.生活质量评分方面,治疗后较治疗前高,差异显著(P<0.05).结论 腓骨肌皮瓣游离移植具有灵活性及可靠性高,其更加符合不同类型的下颌骨缺损修复要求,是当前用于修复下颌骨复合组织缺损的常用方法.

    • 摘要: 目的:探讨股前外侧皮瓣携带肌瓣和/或脂肪瓣在口腔颌面部缺损重建中的应用.方法:收集自2014年1月—2018年12月,连云港市第一人民医院口腔科应用股前外侧皮瓣携带肌瓣和/或脂肪瓣修复口腔颌面部肿瘤切除术后的复杂缺损20例,记录皮瓣的类型与面积,供区、受区并发症,患者术后生存质量等情况.结果:所有皮瓣均存活,术后未发生血管危象.术后1例颊癌患者因涎瘘出现伤口积液,另1例出现口底瘘,换药后伤口均愈合.供区伤口仅留下线性瘢痕,术后1例出现伤口积液,经换药后伤口愈合,无感染、血肿等并发症出现.随访3~24个月,患者对面部外形及功能恢复满意.结论:股前外侧皮瓣及其嵌合皮瓣可用于填塞口腔颌面部肿瘤切除术后留下的各种死腔,覆盖钛板及颈部大血管等,能有效地预防和减少术后并发症的发生.

    • 摘要: 颌面骨缺损的再生修复是外科临床亟待解决的问题,目前主要采用的方法有自体移植、异体移植以及人工合成替代物修补的方法.3D打印技术凭借其快速成型、设计精准、个性化等优点,在替代物制造与加工、治疗方案设计与预演、术前指导等方面发挥着重要的作用.通过采用不同材料、不同制造技术获得具有良好机械性能和生物相容性的模具、种植体、移植物,可以进行骨骼和软组织的修补及诱导再生.随着3D打印技术的日益发展,可选择的生物材料种类越来越多,组织工程支架的研究也成为研发的热点.本文就近年来国内外应用于颌面部骨缺损的3D打印材料的研究及发展状况进行总结,并展望3D打印技术的发展前景.

    • 摘要: 近年来3D打印技术发展迅速,首先是在工程领域发展迅速,然后在医学领域也处于快速发展阶段.由于颅颌面部解剖结构复杂,在外伤、先天畸形以及颌面部手术时,恢复颌面部解剖结构对于颌面外科医师极具挑战.目前,临床已将3D打印技术运用于颌面外科的修复重建中,包括颌面部恶性肿瘤手术、颌骨重建、正颌外科、眶底手术以及植入物导板等领域,且取得了较为满意的效果.未来,3D打印技术有望推进外科技术的发展,特别是颌面外科.

    • 摘要: 目的 探讨个性化设计的前臂皮瓣在颌面部缺损修复的方法及疗效. 方法 2011年3月-2015年9月收治的口腔颌面部肿瘤及面部组织缺损患者16例,男8例,女8例.年龄41~70岁,平均54岁.根据每个患者肿瘤切除后缺损的形状、大小,设计出不同形状的前臂皮瓣进行修复,术后观察移植前臂皮瓣成活情况、患者的面部外形恢复情况及语言、进食等功能恢复情况. 结果 术后本组患者面颈部及前臂处切口均Ⅰ期愈合,16例移植前臂皮瓣均成活.所有患者均获随访,随访时间8~36个月.肿瘤无复发,患者语言、进食、咀嚼等功能均基本正常,面部外形基本对称,患者对自己术后外形及功能恢复均较满意. 结论 根据颌面部的软组织缺损采取个性化的方式进行前臂皮瓣设计,能够最大程度的恢复患者的面部外形及功能,提高患者术后的生存质量,可以获得满意的修复效果.%Objective To investigate the forearm flap personalized design method and the curative effect in repairing maxillofacial defects.Methods From March,2011 to September,2015,16 cases with oral maxillofacial tumor and facial tissue defect were treated.There were 8 males and 8 females with an age range of 41-70 years (mean,54 years).According to the shape and size of the defect after tumor resection in each patient,the forearm flap was designed to repair the defect.The effectiveness was evaluated after operation by observing the survival of the forearm flap transplantation,the patient's facial appearance,the functions of speech,swallowing.Results The incision healed by first intention in this group,16 cases of forearm flap survived.All cases were followed-up 8-36 months without tumor recurrence.The functions of speech,swallowing were basically normal.The facial appearance was satisfactory.The patients were satisfied with their appearance and functional recovery after operation.Conclusion Take personal way of forearm flap design according to maxillofacial soft tissue defect,can restore the maximum facial appearance and function of patients,improve the quality of life of patients after surgery to repair,can obtain satisfactory results.

    • 摘要: 目的 探讨股前外侧组织瓣在颌面部较大缺损及复杂缺损修复的应用效果. 方法 对2008年1月至2016年3月南京大学附属口腔医院口腔颌面外科422例应用股前外侧组织瓣同期修复颌面部恶性肿瘤切除术后较大缺损的临床病例进行分析,其中男258例,女164例,平均年龄57岁,术后观察随访3~84个月.结果 422例移植皮瓣416例成活,成活率为98.6%,其中10例术后血肿,11例静脉血栓,均抢救成功,皮瓣成活,6例皮瓣完全坏死,病例随访3~84个月,平均36.5个月. 结论 股前外侧组织瓣具有供区隐蔽、皮瓣血管蒂较长、管径与颌面部血管管径匹配、可获得皮瓣组织量大、皮瓣柔软可塑性强等优点,是颌面部较大的复杂缺损修复的理想选择.%Objective To explore the value of free anterolateral thigh flaps in the reconstruction of maxillofacial defects after resection.Methods Patients were recruited from January,2008 to March,2016 in the Department of Oral and Maxillofacial Surgery of Nanjing University.All patients sunder when reconstructive surgery employing anterolateral thigh myocutaneous flaps follow-up of 3 months to 84 months after operation.Results Among 422 cases,survived completely with satisfied configuration,the survival rate was 98.6%.Postoerative vessel thrombosis occurred in flaps,21 patients required operative exploration in the perioperative period including 10 flaps with thrombotic events and 11 flaps with venous thrombosis were complete survival after the salvages,and 6 flaps were failure.The mean follow-up was 36.5 months (range:3 to 84 months).Conclusion The free anterolateral thigh flap is an ideal choice for reconstruction of maxillofacial defects,as it has many advantages such as abundance volume,easily molding and adjusting the thickness,possessing,long vascular pedicle,composition need to be adjusted according to defects,easily concealed donor site,and little donor site complications and malformation.

    • 摘要: 目的 探讨显微修薄股前外侧穿支(ALTP)皮瓣在口腔颌面部软组织缺损精细修复重建中的应用并评价其临床效果. 方法 2014年1月-2015年9月,采用显微修薄ALTP皮瓣修复42例口腔癌患者肿瘤根治术后的软组织缺损,术前通过手持式声学多普勒初步确定穿支数目、位置,于髂前上棘与同侧髌骨外侧缘连线中点附近穿支血管标记点内侧纵行切开皮肤及皮下,使用手术显微镜或放大镜于阔筋膜上寻找、解剖穿支血管并确认其走行方式与类型,根据原发灶切除后组织缺损的范围设计皮瓣的大小,同时依据缺损的深度或拟重建器官的三维形态,对皮瓣进行个性化修薄,精确修复组织缺损、重建器官形态. 结果 42例患者共采用45块显微修薄股前外侧穿支皮瓣修复舌、颊、口底及牙龈等部位的软组织缺损均获得成功,其中28块含单一穿支血管,14块含2条穿支血管,3块含3条穿支血管.直接皮支型穿支28条,筋膜皮支型穿支13条,筋膜支型穿支24条.1例患者术后6h出现颈部血肿,再次手术清除血肿结扎出血点后皮瓣成活,4例患者皮瓣边缘部分坏死,积极清创换药后愈合.术后随访6~24个月,皮瓣受区外形满意,供区切口瘢痕细微,患者语音及吞咽功能恢复良好. 结论 显微修薄ALTP皮瓣能个性化修复舌、颊、口底等部位软组织缺损,较好地恢复其外形及功能,且供区损伤小,是精细修复口腔颌面部软组织缺损的理想方法.%Objective To investigate the application of microdissected thin anterior lateral thigh perforator flap (ALTP flap) in precise reconstruction of oral and maxillofacial soft tissue defects and to assess the clinical reconstruction outcome.Methods From January,2014 to September,2015,utilizing the technique of microdissected thin anterior lateral thigh flaps to repair the defects resulted from radical resections of primary sites in 42 patients with oral cancer.Before the operation,the dominant perforator was detected and marked on the skin where the planned flap would be harvested later by Doppler probing.Initial incision was made medial to the perforator marker.Careful dissection was conducted suprafascially to isolate the dominant perforator as well as identifying its source and type.The size of flap was determined according to the area of defect after tumor ablation.Meanwhile the flap was thinned in accordance with the contour of the defect or the organ to be reconstructed.The procedure should be performed under operating microscope.Results Forty-five microdissected thin ALTP flaps were harvested and used to reconstruct the defects of tongue,buccal,oral floor,and gingiva in 42 patients.Among them,28flaps were nourished by single perforator,14 flaps were nourished by 2 perforators,and 3 flaps were nourished by 3 perforators.In total,65 perforators were identified,in which 28 were direct cutaneous branches,13 were fascia-cutaneous branches,and 24 were fascia branches.One take-back occurred within 6 hours due to cervical haematoma.During re-exploration,pedicle compression was found.And the flap was salvaged successfully after haemotoma removement and proper bleeding point ligation.Four flaps underwent marginal necrosis which healed smoothly after debridement and dressing changes.Post-operative 6 to 24 months' follow-up showed satisfactory appearance of the recipient sites with favorable speech and swallowing function.All donor sites were closed primarily,leaving no donor-site complications including incision disruption,scar hyperplasia or muscle strength degeneration of the lower limb.Conclusion Microdissected thin anterior lateral thigh perforator flap is an optimal choice for personalized reconstruction of soft tissue defects of the tongue,buccal,oral floor,and gingiva,for it can decrease donor-site morbidity and achieve better appearance and function reconstruction effect of the recipient site.
