    • 摘要: Agomelatine is a selective agonist of melatonin receptor 1A/melatonin receptor 1B(MT/MT)and antagonist of 5-hydroxytryptamine 2C receptors.It is used clinically to treat major depressive episodes in adults.The pro-chronobiological activity of agomelatine reconstructs sleep-wake rhythms and normalizes circadian disturbances via its agonistic effect of melatonin receptor 1A/melatonin receptor 1B,which work simultaneously to counteract depression and anxiety disorder.Moreover,by antagonizing neocortical postsynaptic 5-hydroxytryptamine 2C receptors,agomelatine enhances the release of dopamine and noradrenaline in the prefrontal cortex,increases the activity of dopamine and noradrenaline,and thereby reduces depression and anxiety disorder.The combination of these two effects means that agomelatine exhibits a unique pharmacological role in the treatment of depression,anxiety,and disturbance of the circadian rhythm.Emotion and sleep are closely related to memory and cognitive function.Memory disorder is defined as any forms of memory abnormality,which is typically evident in a broad range of neurodegenerative diseases,including Alzheimer’s disease.Memory impairment and cognitive impairment are common symptoms of neurodegenerative and psychiatric diseases.Therefore,whether agomelatine can improve memory and cognitive behaviors if used for alleviating depression and circadian-rhythm sleep disorders has become a research“hotspot”.This review presents the latest findings on the effects of agomelatine in the treatment of psychologic and circadian-rhythm sleep disorders in clinical trials and animal experiments.Our review evaluates recent studies on treatment of memory impairment and cognitive impairment in neurodegenerative and psychiatric diseases.

    • 摘要: Objective:To investigate the effect of psychological education combined with progressive muscle relaxation training on the symptom cluster and rehospitalization of elderly patients with coronary heart disease.Methods:This study is a longitudinal randomized controlled study involving 140 elderly patients with coronary heart disease.The patients were divided into two groups:an intervention group and a control group,with 70 cases in each group,via random number table.The patients in the control group received routine nursing,whereas those in the intervention group received psychological education combined with progressive muscle relaxation training for 4 weeks on the basis of routine nursing.The effect of the intervention was evaluated before intervention,at the end of 1 month,3 months,and 6 months after intervention.Results:At the end of 1 month,3 months,and 6 months,the sleep,fatigue,anxiety,and functional status of the patients in the intervention group were significantly better than those of the control group,with statistical significance difference(P<0.05).The rehospitalization rate of the intervention group was lower than that of the control group,and the difference was statistically significant(X2=10.685,P=0.001).Conclusion:Psychological education combined with progressive muscle relaxation training is effective in alleviating the symptom cluster of elderly patients with coronary heart disease and reducing their rehospitalization rate;thus,it should be popularized.

    • 摘要: BACKGROUND The coronavirus disease 2019(COVID-19)pandemic altered education,exams,and residency applications for United States medical students.AIM To determine the specific impact of the pandemic on US medical students and its correlation to their anxiety levels.METHODS An 81-question survey was distributed via email,Facebook and social media groups using REDCapTM.To investigate risk factors associated with elevated anxiety level,we dichotomized the 1-10 anxiety score into low(≤5)and high(≥6).This cut point represents the 25th percentile.There were 90(29%)shown as low anxiety and 219(71%)as high anxiety.For descriptive analyses,we used contingency tables by anxiety categories for categorical measurements with chi square test,or mean±STD for continuous measurements followed by t-test or Wilcoxson rank sum test depending on data normality.Least Absolute Shrinkage and Selection Operator was used to select important predictors for the final multivariate model.Hierarchical Poisson regression model was used to fit the final multivariate model by considering the nested data structure of students clustered within State.RESULTS 397 medical students from 29 states were analyzed.Approximately half of respondents reported feeling depressed since the pandemic onset.62%of participants rated 7 or higher out of 10 when asked about anxiety levels.Stressors correlated with higher anxiety scores included“concern about being unable to complete exams or rotations if contracting COVID-19”(RR 1.34;95%CI:1.05-1.72,P=0.02)and the use of mental health services such as a“psychiatrist”(RR 1.18;95%CI:1.01-1.3,P=0.04).However,those students living in cities that limited restaurant operations to exclusively takeout or delivery as the only measure of implementing social distancing(RR 0.64;95%CI:0.49-0.82,P<0.01)and those who selected“does not apply”for financial assistance available if needed(RR 0.83;95%CI:0.66-0.98,P=0.03)were less likely to have a high anxiety.CONCLUSION COVID-19 significantly impacted medical students in numerous ways.Medical student education and clinical readiness were reduced,and anxiety levels increased.It is vital that medical students receive support as they become physicians.Further research should be conducted on training medical students in telemedicine to better prepare students in the future for pandemic planning and virtual healthcare.

    • 摘要: Background: While causing a financial and economic loss in many countries in a few months, COVID 19 was of great impact on public health. The Iraqi government took several measures since the first Iraqi case was discovered in February 2020. One of the fundamental measures to control this pandemic is social isolation, which depends greatly on people’s awareness to decrease the incidence of COVID-19. Objectives: To study the COVID-19-effect on dentists’ social life and their future plan, and to find the relation between that effect and some dentists’ demographic variables. Methods: A cross-sectional study was conducted from 2nd January to 7th May 2021, by an electronic version of the questionnaire through Google form. With facilitating orders delivered to all the specialist dental centers for adults of Al-Resafa health directorate;any dentists working in specialist dental centers for adults of Al-Resafa health directorate, and accept to participate in this study. Results: A total of 10 specialist dental centers for adults of the Al-Resafa health directorate represent 566/628 dentists enrolled in this study with a response rate of 88.99%. Most of them aged less than 30 years old (62.2%), female (60.6%), married status (50.4%), had 1 - 3 children (71.91%), Rotator (51.1%), with less than five years of experience (61.3%). Conclusion: COVID-19 affected so many aspects of health workers’ lives including dentists. About two-thirds of dentists had negative feelings, of which almost all had anxiety. Younger ages, females, married and having children are factors that contribute to the affection of dental social lives more than other categories.

    • 摘要: Background: Sexual dysfunction is sexual complaints or disorders. There are various causes, but the onset leads to deterioration of relationships and quality of life with partners. There is still a deep-rooted awareness that sexuality is a secret in Japan. Aim: This study aims to clarify the relationship between female sexual function and depression or anxiety. Method: Four hundred and fifty-eight Japanese healthy women who worked in four randomly selected medical institutions in Okinawa Prefecture in Japan from March to May 2012 were asked to agree to a consent form of their own free will and participate in self-reported questionnaires. For their evaluation, the Female Sexual Function Index (FSFI;Rosen et al. 2000, which includes the six domains of desire, arousal, lubrication, orgasm, pain, and satisfaction) and the Kessler Psychological Distress Scale (K6;Kessler, 2002, which includes the following six items: felt nervous, hopeless, restless or fidgety, worthless, depressed, and felt that everything was an effort) was used. Moreover, participants’ characteristics showed age, marriage, menopause, annual income, and drinking. In addition, JMP16.2 and R 4.2.0 was used to perform Pearson’s chi-square test, Fisher’s exact test, Mann-Whitney U test, Kruskal-Wallis test, Steel-Dwass test, Spearman’s Rank-Order Correlation, and Cronbach’s alpha. This study was approved by the university and the medical institutional review board (IRB). Results: 178 women were included in the final analysis, and the median (IQR, interquartile range) was 39 (32 - 48) years old, and the mean ± SD (standard deviation) was 40.2 ± 10.4 years old. The median (IQR) and mean ± SD of FSFI Total Score were 22.0 (9.3 - 26.6) and 19.2 ± 9.6. The median (IQR) and the mean ± SD of K6 Total Score were 3 (0 - 7) and 4.2 ± 4.5. 16% of all women with a K6 Total Score of 10 and more, and 5% of all women with a K6 Total Score of 13 and more considered a serious mental illness. There was no clear association between female sexual function and depression or anxiety in all health worker participants between FSFI Total Score and K6 Total Score by Spearman’s Rank-Order Correlation Coefficient (ρ). However, there was a moderate correlation between the K6 Total Score and the FSFI Total Score in health worker participants with a K6 Total Score of 11 or more (n =19, ρ = ?0.62, P = 0.005). Moreover, there was a weak correlation between them in both married and drinking women or married and non-menopausal women. In the Mann-Whitney U test where few participants had morbid depression, significant relationships were found in the association between a sexual function with high depression or anxiety, whereas the low group had a K6 cut-off value of 11. Conclusion: Female sexual morbidity for high depression or anxiety should be managed with interventions. Especially, the intervention for decreasing female sexual function in the K6 Total Score of 11 or more will be needed, and a randomized study is required for more evidence.

    • 摘要: BACKGROUND Parental drinking has a direct bearing on children.Behavioral problems such as anxiety and depression are common problems among children whose parents drink heavily.Psychosocial interventions have shown promising results for anxiety and depression among children;however,few studies have been conducted in the context of children of parents with alcohol dependence in India.AIM To evaluate the efficacy of psychosocial intervention for internalizing behavioral problems among children of parents with alcohol dependence.METHODS A randomized controlled trial with a 2×4 factorial design was adopted with longitudinal measurement of outcomes for 6 mo.Two-hundred and eleven children who met the eligibility criteria(at least one parent with alcohol dependence)at government high schools in Bangalore,India,were randomized to the experimental(n=97)or control group(n=98).The psychosocial intervention was administered to the experimental group in eight sessions(biweekly)over 4 wk after baseline assessment.The intervention focused on identifying and modifying negative thoughts,replacing thinking errors with realistic alternatives,modification of maladaptive behavior,developing adaptive coping skills and building self-esteem.The data was collected pre-intervention and at 1,3 and 6 mo after the intervention.Data were analyzed using SPSS 28.0 version.RESULTS Mean age of the children was 14.68±0.58 years,60.5%were male,56%were studying in 9th standard,70.75%were from nuclear families,and mean family monthly income was 9588.1±3135.2 INR.Mean duration of parental alcohol dependence was 7.52±2.94 years and the father was the alcohol-consuming parent.The findings showed that there were significant psychosocial intervention effects in terms of decreasing anxiety and depression scores,and increasing selfesteem level among experimental group subjects over the 6-mo interval,when compared with the control group(P<0.001).CONCLUSION The present study demonstrated that the psychosocial intervention was effective in reducing anxiety and depression,and increasing self-esteem among children of parents with alcohol dependence.The study recommends the need for ongoing psychosocial intervention for these children.

    • 摘要: Objective:The clinical efficacy and safety of traditional Chinese medicine in the treatment of anxiety and/or depression after PCI for coronary heart disease were systematically evaluated.Methods:The databases of CNKI,Wanfang,VIP,CMB,PubMed,Web of Science,PubMed,Web of Science and Cochrane Library were searched by computer,and the Chinese and English randomized controlled trials(RCTs)of traditional Chinese medicine in the treatment of anxiety and/or depression after PCI of coronary heart disease published from the database to November 2020 were included.The Cochrane bias risk tool and the modified Jadad scale were used to evaluate the quality of the included literature,and the RevMan 5.3 and Stata 16.0 software were used for Meta-analysis.Results:A total of 23 RCTs were included,including 1837 subjects,including 917 cases in the treatment group of traditional Chinese medicine,920 cases in the control group,the baseline situation between groups was consistent,comparable.Meta-analysis results show that TCM treatment improves the total clinical effective rate[OR=2.34,95%CI(1.61,3.40),P<0.00001],and the efficacy of TCM syndrome[OR=5.19,95%CI(2.74,9.86),P<0.00001],reduce Hamilton anxiety scale HAMA score[SMD=-0.96,95%CI(-1.78,-0.14),P=0.02],Hamilton Depression Scale HAMD score[SMD=-1.10,95%CI(-1.79,-0.41),P=0.002],SAS self-rating scale score[SMD=-1.20,95%CI(-1.75,-0.65),P<0.0001],SDS self-rating scale score[SMD=-1.14,95%CI(-1.62,-0.65),P<0.00001]and other indicators are better than the control group,and the difference is statistically significant(P<0.05).Conclusion:The combination of traditional Chinese medicine intervention and conventional western medicine treatment after PCI can further improve the clinical efficacy of patients with anxiety and/or depression after PCI of coronary heart disease and has good safety.Due to the general quality of the literature included in this study,the above conclusions need to be further confirmed by high-quality randomized controlled double-blind clinical trials.

    • 摘要: Tea(Camellia sinensis)is widely considered to promote feelings of calming and soothing.This effect is attributed to L-theanine(L-γ-glutamylethylamide)in tea,a non-protein amino acid mainly derived from tea leaves.As a naturally occurring structural analogue of glutamate,L-theanine competes for the receptors with glutamate and is able to pass the blood-brain barrier to exert its relaxation effect.This review focuses on the relaxation effect of L-theanine,including animal models and the latest human trials as well as the potential molecular mechanisms regarding neuron stem cells.The biological efficacy of dietary L-theanine in the food matrix has been further discussed in this review in relation to the physiological changes in the gastrointestinal tract and bindings of L-theanine with other food components.

    • 摘要: Chronic intake of High Fat Diet (HFD) has the potential of causing a number of metabolic disorders. It is also theorized to be involved in perturbation of gut microbiota. Cannabididol (CBD) and omega 3 are known to possess a number of medicinal usefulness. Their combined use in experimental interventions is quite limited. A total of 15 mice were used for this research divided into three groups of five animals each. Group 1 was administered water and normal chow ad libidum. Group 2 had HFD and water ad libidum. Group 3 had HFD plus CBD (10 mg/kg) and omega 3 (200 mg/kg) all for a total of 12 weeks. They were tested on the elevated plus maze (EPM) and average entry time into the closed arm was recorded. They were also tested on the Y-maze and spontaneous alternations were measured. Thereafter animals were sacrificed and faecal content in the caecum was collected in sterile bottles and cultured for E. coli count. It was found that HFD group at p value 6 ± 4.5) compared to group 1 (1.4 × 106 ± 5.6) and group 3 (1.42 × 106 ± 6.3). The findings revealed that HFD enhanced gut E. coli overgrowth which was reduced by CBD and Omega 3. The memory impairment and anxiety induction by HFD was also significantly ameliorated by CBD and omega 3. E. coli known to be implicated in dementia induction was suppressed by the interventions. Possible mechanisms proposed are actions of CBD and omega 3 on CB1, TRVP and 5HT receptors in reducing anxiety and their antioxidant/anti-inflammatory actions in mitigating the neuro-inflammatory effect of HFD and immune hyperstimulation of E. coli via the gutbrain-axis.
