摘要:
目的:探究圆锥型套筒冠义齿对牙周病伴牙列缺损患者咀嚼效能、菌斑指数( PLI)及牙齿松动度的影响.方法:176例牙周病伴牙列缺损患者根据自愿选择的修复方法不同分为A组(圆锥型套筒冠义齿修复,n=96) 、B组(传统可摘义齿修复,n=80) .修复后1 个月、6 个月、1 年采用过筛法评估修复体咀嚼效能,评价患者PLI、牙龈指数( GI)及牙齿松动度,并进行自觉满意度调查.结果:两组患者修复前咀嚼效能均较差,差异无统计学意义( P>0.05) ,修复后1年内咀嚼效能均逐渐改善,且A组修复后1个月、6个月、1年咀嚼效能明显优于B组(P<0.05).两组患者修复前 PLI、GI 及牙齿松动度接近,差异无统计学意义( P>0.05) ,修复后PLI、GI 及牙齿松动度均明显降低,且 A 组修复后降低幅度均明显大于B组( P<0.05) .修复治疗后1年,A组患者舒适感、咀嚼能力、语音能力及固位能力自觉满意度明显高于B组,差异有统计学意义( P<0.05) .结论:与传统可摘义齿相比,圆锥型套筒冠义齿修复牙周病伴牙列缺损可有效提升患牙咀嚼效能,降低PLI及牙齿松动度,对改善患者各方面自觉满意度有更积极的作用.%Objective: To explore the effects of conical telescopic denture on masticatory efficiency, plaque index ( PLI) and tooth mobility in patients with periodontal disease and dentition defect. Methods:176 patients with periodontal disease and dentition defect were divided into group A ( treated with conical telescop-ic denture, n=96) and group B ( treated with traditional removable denture, n=80) according to different re-pair methods. The masticatory efficiency of the prosthesis was evaluated by sieving at 1 month, 6 months and 1 year after the repair. The PLI, gingival index ( GI) and tooth mobility were evaluated, and the satisfaction de-gree was investigated. Results: The masticatory efficiency of two groups was poor before repair ( P > 0.05) . It was gradually improved in 1 year after repair, and the masticatory efficiency of group A was significantly better than that of group B at 1 month, 6 months and 1 year after repair ( P <0.05) . Before repair, the PLI, GI and tooth mobility were similar in the two groups ( P > 0.05) . After repair, the PLI, GI and tooth mobility were significantly decreased, and the decrease in group A was significantly greater than that in group B ( P < 0. 05) . In 1 year after repair, the satisfaction degree of comfort, chewing ability, phonological competence and retention ability of group A was significantly higher than that of group B ( P < 0.05) . Conclusion: Compared with the traditional removable denture, conical telescopic denture can effectively improve the masticatory effi-ciency, reduce PLI and tooth mobility, and improve the satisfaction of patients with periodontal disease and dentition defect.
