顺尔宁乌司他丁与米力农联合治疗对小儿重症肺炎的影响

             

摘要

目的 探究顺尔宁、乌司他丁与米力农联合治疗对小儿重症肺炎氧化应激反应、血气分析和心肌酶谱的影响及机制,为小儿重症肺炎的临床治疗提供帮助.方法 选取84例重症肺炎患儿,按随机数字表法分为观察组和对照组,每组各42例.对照组患儿给予基本治疗,观察组患儿在此基础上给予顺尔宁、乌司他丁与米力农的联合治疗,检测两组患儿治疗前后氧化应激反应、血气分析和心肌酶谱指标的变化情况,并分析相关机制.结果 治疗前两组重症肺炎患儿氧化应激反应、血气分析和心肌酶谱指标水平比较差异无统计学意义(P>0.05).与治疗前比较,经过相关治疗后两组患儿丙二醛(MDA)、心肌酶谱指标和动脉血二氧化碳分压(PaCO2)显著降低,氧化应激反应指标和血气分析指标显著升高,差异有统计学意义(P<0.05).经过联合治疗后观察组患儿氧化应激反应指标和血气分析指标显著高于治疗后对照组,MDA、心肌酶谱指标和PaCO2显著低于治疗后对照组,差异存在统计学意义(P<0.05).结论 顺尔宁、乌司他丁与米力农的联合治疗能够抑制重症肺炎患儿的氧化应激反应,提高患儿的血氧水平同时改善心肌状况,对重症肺炎患儿的临床治疗具有重要的意义.%Objective To observe the effect of singulair,ulinastatin combined with milrinone on oxidative stress index,blood gas analysis and myocardial enzyme spectrum in children with severe pneumonia and offer help for children with severe pneumonia treatment. Methods A total of 84 severe pneumonia children were selected and randomly divided into two groups:the research group(42 patients)and the control group(42 patients). The patients in the control group were treated with basic treatment and the patients in the research group were treated with singulair, ulinastatin combined with milrinone. Oxidative stress index,blood gas analysis and myocardial enzyme spectrum were compared before and after therapy. Results Before treatment,oxidative stress index,pneumonia effect analysis and myocardial enzyme spectrum of two groups had no statistically significant difference comparison. MDA,myocardial enzyme spectrum and PaCO2 of two groups decreased significantly than before treatment(P<0.05). Oxidative stress index and pneumonia effect analysis of two groups increased significantly than that before treatment(P<0.05). Oxidative stress index and blood gas analysis of research group after treatment increased significantly than control group after treatment(P<0.05). MDA,myocardial enzyme spectrum and PaCO2 of research group after treatment increased significantly than control group after treatment(P<0.05). Conclusion Singulair,ulinastatin combined with milrinone can inhibit oxidative stress,improve blood oxygen level,and perfect myocardial condition. It has important clinical significance for severe pneumonia children treatment.
