地佐辛与舒芬太尼对无痛人流术效果及术后宫缩痛的影响

摘要

目的：分析和探讨地佐辛与舒芬太尼对无痛人流术效果及术后宫缩痛的影响。方法选取我院2014年1～5月收治的200例行无痛人流术患者，通过随机数字表法分为舒芬太尼组（S组，n=100）和地佐辛组（D组，n=100），S组采用舒芬太尼麻醉，D组采用地佐辛麻醉，监测患者麻醉前（T0）、意识消失即刻（T1）、扩张宫颈时（T2）及术毕苏醒时（T3）时点的平均动脉压(MAP)、心率(HR)、脉氧饱和度(SpO2)、呼吸（RR）；记录术后5、15、30min宫缩痛的VAS评分及不良反应情况。结果两组患者术中丙泊酚用量，术后5、15min宫缩疼痛比较无统计学意义（P>0.05），D组术后30min宫缩疼痛较S组更轻，术中呼吸抑制发生率低于S组（P<0.05），两组术后恶心、呕吐发生率比较无统计学意义（P>0.05）。但地佐辛组术后清醒时间明显短于舒芬太尼组(P<0.05)。结论在无痛人流术中，地佐辛与舒芬太尼麻醉镇痛效果相似，都能取得满意的麻醉效果，但地佐辛术后清醒时间短，而且呼吸抑制发生率低于舒芬太尼，还能够持续减轻术后宫缩痛程度，有效减轻患者生理和心理痛苦，提高患者生活质量，安全可行，值得临床推广应用。%Objective To probe into and analyze the effects of painless abortion and uterinecon-traction pain of dezocine and sufentanil. Methods 200 patients with painless artificial abortion and who accepted treatments in our hospital from January, 2014 to May, 2014 were taken as the research objects, and these patients were randomly divided into the sufentanil group (group S, n=100) and dezocine group(group D, n=100),according to the random number table method. In the S group, they were treated with the sufentanil anesthesia while in the D group, they were treated with the dezocine anesthesia. Then,before anesthesia (T0), loss of consciousness (T1) immediately, dilatation of the cervix(T2) and postoperative recovery time point(T3), mean arterial pressure (map), heart rate (HR), pulse oxygen saturation (SpO2),respiratory rate (RR); recording after 5min,15min, 30min uterine contraction pain VAS scores and adverse reactions were monitored.Results The op-eration of propofol dosage, after 5min,15min uterine pain of two groups of patients had no statisti-cally significant (P>0.05),thepostoperative 30min uterine pain in the D group was lighter than that in the S group,the intraoperative respiratory inhibition rate lower than s group(P<0.05).The postop-erative nausea and vomiting occurrence rate of two group had no statistically significant (P>0.05) but after waking time in dezocine group was significantly shorter in sufentanil group (P<0.05). Conclusion In painless artificial abortion, dezocine and sufentanil anesthesia analgesic effect similar can obtain satisfactory anesthetic effect, but dezocine awake after a short time, and respi-ratory depression and lower incidence of sufentanil, continued to reduce postoperative uterine con-traction pain, relieve suffering from physical and psychological suffering and improve the quality of life of patients,so it is safe and feasible,thus should be promoted in the clinical application.
