百强固位纤维用于前牙外伤松动牙的效果及对牙龈指数和龈沟出血指数的影响

摘要

Objective To investigate the effect of biosplint ribbons on the treatment of anterior teeth injury,and to analyze its effect on gingival index ( GI) and gingival sulcus bleeding index ( SBI) of the patients.Methods The clinical data of 108 patients(164 teeth) treated in Shanghai Fifth People′s Hospital of from Jun.2012 to Jun.2014 were retrospectively analyzed.According to the treatment to the loosened teeth,the 108 patients were divided into three groups:group A(biosplint ribbons with flow resin,38 cases,56 teeth),group B(steel wire with resin,34 cases,53 teeth),group C(dental arch splint,36 cases,55 teeth). The therapeutic effect of the three groups was observed,GI and SBI were used to evaluate the effects of the three fixation methods on the gingiva and the surrounding healthy teeth .Results The efficacy after 6 months of group A was significantly higher than group C[ 92.1%(35/38) vs 61.1%(22/36)](P<0.05).And there was no significant difference between group B[ 85.4%(28/34)] and group A(P<0.05).The oral GI score of all three groups increased 6 weeks after treatment,and declined 6 months later(P<0.05);SBI score of group A and group C increased 6 weeks after treatment,and declined 6 months later(P<0.05),SBI score of group B declined.GI,SBI score between groups,different time points,between groups· time points interaction hadstatistically significant differences ( P <0.05 ) .Conclusion The biosplint ribbon with flow resin fixation has significant and stable efficacy for loosened anterior teeth due to injury ,with little influence on the gingiva and the surrounding healthy teeth,thus is worthy of clinical promotion.%目的：探讨百强固位纤维治疗前牙外伤松动牙的效果及对牙龈指数（ GI）和龈沟出血指数（ SBI）的影响。方法回顾性分析2012年6月至2014年6月上海市第五人民医院收治的108例患者（164颗患牙）的临床资料，将其按照治疗过程中使用的松动牙固定方法分为 A 组（38例，56颗牙），B组（34例，53颗牙）、C组（36例，55颗牙）。 A 组采用百强固位纤维联合流动树脂，B 组采用钢丝联合树脂，C组采用牙弓夹板。观察三组患者的治疗效果，采用 GI和 SBI评价3种固定方式对牙龈和周围健康牙的影响。结果三组患者拆去固定装置6个月后疗效显示，A组治疗总有效率高于C组[92.1％（35／38）比61.1％（22／36）]，差异有统计学意义（P＜0.05），而与 B 组[85.4％（28／34）]比较差异无统计学意义（P＞0.05）。三组患者口腔GI评分呈治疗后6周升高，治疗后6个月下降的趋势（P＜0.05）；A组和C组SBI评分呈治疗后6周升高，治疗后6个月下降的趋势（P＜0.05）， B组SBI评分呈下降趋势。三组患者GI、SBI评分组间、不同时间点、组间·不同时间点交互作用比较差异有统计学意义（ P＜0.05）。结论百强固位纤维联合流动树脂固定前牙外伤松动牙，疗效显著且稳定，对牙龈和周围健康牙影响小，值得临床推广。
