无创正压机械通气治疗AECOPD合并Ⅱ型呼吸衰竭的临床疗效观察

摘要

Objective To observe the ef icacy of the noninvasive positive pressure mechanical ventilation on AECOPD combined with type Ⅱ respiratory failure. Methods 116 patients with AECOPD combined with type Ⅱ respiratory failure were selected and randomly divided into the observation group (58 cases) and control group (58 cases).The control group were given routine treatment,the observation group were treated with noninvasive positive pressure mechanical ventilation in addition to the routine treatment.The changes of clinical symptoms,arterial blood gas(PaCO2、PaO2),the first second forced expiratory volume (FEV1)and the percentage of FEV1 in predicted values before and after the treatment were compared. Results There were obvious dif erences between the two groups: <0.05,and there was no severe complication.Conclusion The noninvasive positive pressure mechanical ventilation have a good clinical ef icacy on AECOPD combined with type Ⅱrespiratory failure,it is safe and easy to conduct,which deserved promotion.%目的：观察无创正压机械通气治疗AECOPD合并Ⅱ型呼吸衰竭的临床疗效。方法选择AECOPD合并Ⅱ型呼吸衰竭患者116例,随机分为观察组58例和对照组58例,对照组采用传统方法治疗,观察组在传统治疗同时应用无创正压机械通气。比较两组治疗前后患者动脉血气指标(PaO2,PaCO2),肺功能指标(FEV1,FEV1占预计值的百分比)变化,以及治疗效果等。结果两组患者比较差异有统计学意义,<0.05,并且无严重并发症发生。结论在常规治疗基础上应用无创正压机械通气治疗AECOPD合并Ⅱ型呼吸衰竭可以显著提高临床疗效,安全易行,值得推广。
