Changes in Payment Source for Elderly Nursing Home Residents. Executive Summary

The study described changes in payment sources among nursing home residents using a longitudinal data set of all admissions and discharges to a chain of 48 nursing homes covering the period 1982 through 1989. The study analyzed the factors associated with time to conversion to Medicaid. Semi-Markov models were used to study the processes of payment source models were used to study the processes of payment source transition among initially private pay admissions. Nineteen percent of private-payers converted to Medicaid after one year. State policies are strongly related to the rate of conversion to Medicaid. Patients with higher levels of education were less likely to convert to Medicaid with education acting as a proxy for social and economic status.
