联合检测糖化血清蛋白、糖化血红蛋白、脂蛋白(a)及纤维蛋白原评估2型糖尿病合并微血管损伤的临床意义

         

摘要

Objective To study the clinical significance of glycosylated hemoglobin, glycosylated serum protein, Lp( a) , and FIB in patients with type 2 diabetic microangiopathy. Methods Eighty - four patients with diabetes mellitus were divided into group A (44 cases that were complicated with diabetic microangiopathy) and group B (40 cases without complications with diabetic microangiopathy) , and another 40 cases were used as controls. Glycosylated hemoglobin, glycosylated serum protein, Lp(a), and FIB were assayed. Results The level of glycosylated hemoglobin, glycosylated serum protein, Lp(a), and FIB was significantly higher in group with diabetes mellitus than in control group (P<0.05). These parameters were higher in group B than those in group A (P<0.05). Conclusion The combined assay of glycosylated hemoglobin,glycosylated serum protein, Lp( a) , and FIB in diabetic patients plays an important role in the diagnosis and treatment of diabetic microangiopathy, which provides clinical evidence for pre - treatment of mi-crocirculation disturbance in patients with type 2 diabetes.%目的 探讨2型糖尿病患者糖化血红蛋白(HbAlc)、糖化血清蛋白(glycosylated serum protein,GSP)、脂蛋白(a)[ Lipoprotein(a),Lp(a)]及纤维蛋白原(fibrinogen,FIB)水平与微血管病变的关系.方法 将84例2型糖尿病患者分为A、B两组:A组为合并微血血[管病变者44例;B组为无微血管病变者40例;同时选取40例健康对照者,分别检测各组人群的HbA1c、GSP、Lp(a)及FIB,分析以上指标在健康人和糖尿病患者中的差异.结果 糖尿病患者的HbAlc、GSP、Lp(a)及FIB明显高于对照组(P<0.05);有微血管病变组明显高于无微血管病变组(P<0.05).结论 HbAlc、GSP、Lp(a)及FIB的联合检测为2型糖尿病患者微循环障碍的提前诊断及治疗提供临床依据.
