实验性糖尿病酮症酸中毒家兔Ⅰ型肺泡上皮结构和功能变化的研究

         

摘要

Objective : To study the membranous and structure changes of type Ⅰ alveolar cells in diabetic ketoacidosis rabbit models. Methods : 12 male New Zealand rabbits were divided into experimental groups( DK groups,n =6 ) and control groups( NS groups,n =6 ) randomly; streptozocin ( 150 mg· kg-1 ) and alloxan monohydroate ( 150 mg·kg-1 ) each in DK groups were intravenously injected and NS groups were given the same dose of normal sodium chloride. Blood gas, blood electrolyte and urine acetone body were detected ; All the animals were killed 72 hours later, blood gas, alkaline phosphates( ALPase ) of cytohistochemical examination, ultrastructure were detected. Results: Plasma glucose concentration values exceeded 17 mmol·L-1 and urine acetone body appeared in all DK groups, thickening of the alveolar basement membrane and air- blood barrier, inflammatory cellular exudation were shown in DK groups. The activity of alkaline phosphates was lost in type Ⅰ alveolar cells. Type Ⅰ alveolar cell and capillary endothelium damages were observed. No changes were shown in NS groups. Conclusion : DKA induces membranous structures of type Ⅰ alveolar cells to damage.%目的:研究糖尿病酮症酸中毒(DKA)家兔Ⅰ型肺泡上皮结构和功能的变化,探讨其损伤机制.方法:新西兰家兔12只,随机分为实验组(DK组)和对照组(NS组)各6只,DK组家兔予链脲佐菌素(STZ)150 mg·kg-1和四氧嘧啶150 mg·kg-1,NS组予等剂量生理盐水,72 h后检测动脉血气,处死动物取肺标本进行Ⅰ型肺泡上皮超微结构、碱性磷酸酶(ALP)等细胞组织化学检查.结果:(1) DK组血气72 h后皆达到DKA标准.(2) DK组Ⅰ型肺泡上皮超微结构见细胞膜结构模糊、基底膜增厚、细胞膜外聚集高电子物质等损伤.(3) ALP检查:NS组动物ALP活性存在于细胞膜、线粒体上,血管内皮见颗粒状酶反应物;DK组动物细胞膜上ATP活性显著减少,线粒体上酶活性也明显变少.结论:DKA能引起新西兰家兔肺组织Ⅰ型肺泡上皮结构损伤、细胞膜及膜性细胞器功能下降等改变.
