参穹葡萄糖注射液对老年人工髋关节置换全麻术后发生谵妄的影响

摘要

Objective To explore the influence of the dome and glucose injection in elderly hip re-placement anesthesia postoperative delirium.Methods A total of 90 patients who were treated hip re-placement from June 2006 to June 2013 were selected and were randomly divided into 3 grous (each group of 30):the blank group (injection of saline ),the control group(injection of 5% glucose),the re-search group(injection of dome and glucose injection),each group of 250mL.Recorded the HR and MAP of each patient before they received intravenous inj ection(T0 ),at the time of 5 min after the pa-tients received intravenous inj ection(T1 ),at the time of 1 5 min after the patients received intravenous injection(T2),at the time of 30 min after the patients received intravenous injection(T3),and also re-corded the total time (T)of operation from the operation began to pulled out the tracheal catheter.To observated the incidence of postoperative delirium and the adverse drug reaction using the(CAM-CR). Results There were statistically significant in the incidence of postoperative delirium between three groups(P <0.05 ).There were no statistically significant between the blank group and the control group(P >0.05).The incidence of postoperative delirium of the research group(5cases)was more less than blank group(14 cases)and the control group (13 cases)(P <0.05).Conclusion It can effectively prevent the incidence of postoperative delirium in patients with anesthesia,it is helpful for patients re-habilitation.%①目的探讨参穹葡萄糖注射液对老年髋关节置换全麻术后发生谵妄的影响。②方法选择2013年10月～2014年4月本院全麻下行髋关节置换术的老年患者90例，随机分成空白组、对照组、参穹组，各30例。气管插管成功、患者生命体征平稳后，空白组给予生理盐水、对照组给予5％葡萄糖注射液、参穹组给予参穹葡萄糖注射液，各250mL。在静脉输注前（T0），静脉输注后5分钟（T1）、15分钟（T2）、30分钟（T3）记录患者的心率和平均动脉压。记录患者从手术开始到术后拔管的总时间（T）。观察三组患者术后谵妄的发生情况以及药物的副作用。③结果空白组术后发生谵妄14例，对照组13例，参穹组5例。三组两两比较，参穹组与空白组、参穹组与对照组，差异均有统计学意义（P ＜0．05），而空白组与对照组比较差异无统计学意义（P ＞0．05）。④结论术中应用参穹葡萄糖注射液能有效预防老年髋关节置换全麻术后患者谵妄的发生，减轻患者痛苦，有利于患者康复，改善预后。
