低浓度罗哌卡因硬膜外麻醉在肛门直肠手术中的应用观察

摘要

目的 探讨低浓度罗哌卡因硬膜外麻醉在肛门直肠手术中的麻醉效果及应用价值.方法 将120例肛门直肠手术患者随机分为硬膜外阻滞组(EA组)、骶管阻滞组(S1组)、蛛网膜下腔阻滞组(S2组)各40例,对比观察三组患者麻醉镇痛效果、下肢肌力、肛门松弛程度及不良反应.结果 三组相比较,EA组和S1组麻醉镇痛效果及肛门松弛程度差异有统计学意义(P<0.05),EA组和S2组下肢肌力差异有统计学意义(P<0.05),三组均无严重不良反应发生.结论 低浓度罗哌卡因硬膜外麻醉在肛门直肠手术中麻醉镇痛效果可靠,下肢肌力正常,肛门松弛良好,不良反应少,值得推广应用.%Objective To investigate the anesthesia effect and application value of the low concentration ropivacaine epidural anesthesia in anorectal operation. Methods 120 cases of anorectal operation patient were divided into three group randomly. There were 40 cases in Epidural anesthesia group (group EA) , caudal block group (group S1) , suharachnoid space block anesthesia group (group S2) , respectively. Compared with three groups of patients narcotic analgesic effect, muscle strength of lower limbs, anal relaxation degree and adverse reaction. Results According with the analysis of three group, the anesthesia analgesia and anal relaxation extent of group EA and group S1 has difference { P ＜0. 05) , the lower extremity muscle of EA group and S2 group of has statistics difference ( P ＜0. 05). There was no serious adverse reaction in three groups. Conclusion It is reliable that use the low concentration ropivacaine epidural anesthesia in anorectal operation, lower extremity muscle strength is normal, good anal relaxation, fewer adverse reactions. It is worthy of popularization and application.
