21 468例药物咨询的帕累托图分析

             

摘要

OBJECTIVE: To investigate the drug consultation status an questions in People's Hospital of Peking University (hereinafter referred to as "our hospital"). METHODS: A total of 21 468 cases of drug consultation records in our hospital were selected in 2017. The composition of consultants, the questions of consultation and the content of drug consultation were analyzed, and the Pareto chart was drawn. Meanwhile, the main, secondary and general factors were analyzed. RESULTS: Among the drug consultants, there were 8 907 males (41.49%) and 12 561 females (58.51%), more females than males. There were 6 702 cases (31.22%) of telephone consultation and 14 766 cases (68.78%) of on-site consultation, and on-site consultation was the main part. The main consultation questions were basic information, usage and dosage, special population medication, special dosage form and medication precautions and so on; Pareto diagram analysis results showed that basic information, usage and dosage, special dosage form, special population medication and medication precautions were the main factors, drug selection, patient education and drug interaction were the secondary factors. The content of drug consultation were mainly cardiovascular system drugs, respiratory system drugs, anti-infective drugs, central nervous system drugs and specialist drugs and so on; Pareto chart analysis results showed that cardiovascular system drugs, respiratory system drugs, anti-infective drugs, central nervous system drugs and specialist drugs were the main factors, hormones and related drugs, drugs that affect allergy and immune function were the secondary factors. CONCLUSIONS: Pharmacists provide drug counseling service for patients is helpful for the clinical rational drug use. Consulting pharmacists should strengthen the professional learning, improve the pharmaceutical service skills in a more targeted way, provide better pharmaceutical service, strengthen communication with clinicians and nurses and answer questions about medication timely, so as to promote the rational clinical drug use.%目的:了解北京大学人民医院 (以下简称"我院") 药物咨询情况及咨询问题.方法:筛选2017年我院药物咨询记录, 累计21 468例, 对咨询人员构成、咨询问题内容和咨询药品内容等进行分析, 绘制帕累托图, 同时分析主要、次要及一般因素.结果:药物咨询人员中, 男性8 907例 (占41.49%), 女性12 561例 (占58.51%), 女性多于男性;电话咨询6 702例 (占31.22%), 现场咨询14 766例 (占68.78%), 以现场咨询为主.咨询问题内容主要为基本信息、用法与用量、特殊人群用药、特殊剂型指导及用药注意事项等;帕累托图分析结果显示, 基本信息、用法与用量、特殊剂型指导、特殊人群用药及用药注意事项等为主要因素, 遴选药品、患者教育及药物相互作用为次要因素.咨询药品内容主要为心血管系统用药、呼吸系统用药、抗感染药、中枢神经系统用药及专科用药等;帕累托图分析结果显示, 心血管系统用药、呼吸系统用药、抗感染药、中枢神经系统用药及专科用药等为主要因素, 激素及其有关药物、影响变态反应和免疫功能的药物为次要因素.结论:药师为患者提供药物咨询服务, 有助于临床合理用药.咨询药师应加强业务学习, 更有针对性地提升药学服务技能, 提供更优质的药学服务, 加强与医师及护士的沟通和交流, 解答用药问题, 促进临床合理用药.
