目的：观察和比较甘露醇及尼莫地平对高血压脑出血患者血肿体积及神经功能恢复的影响。方法选取86例高血压脑出血患者并采用信封法随机分为2组，所有患者均采用对症支持治疗及复方丹参注射液治疗，对照组患者配合甘露醇降颅内压，观察组采用尼莫地平降颅内压治疗，对2组患者治疗前、治疗后2周及3周进行神经功能缺损评分及血肿测定，并进行疗效评定。结果2组患者治疗前血肿体积无显著差异，治疗后2周及3周时观察组血肿体积明显小于对照组（P＜0.05），2组治疗前神经功能缺损评分无显著差异，但治疗后2周及3周观察组神经功能缺损评分明显低于对照组（P＜0.05），观察组总有效率86.05％，对照组为65.12％，观察组疗效明显优于对照组（ P＜0.05）。结论对高血压脑出血采取基础对症支持治疗的同时，配合尼莫地平降颅内压治疗可明显缩小血肿体积，促进患者神经功能的恢复。%Objective To observe and compare the effects of mannitol and nimodipine on hematoma volume and neurologi-cal function of hypertensive patients with cerebral hemorrhage.Methods 86 cases of hypertensive cerebral hemorrhage were chose and randomly divided into two groups by use of envelopes ;all patients were treated with symptomatic and supportive treatment and Salvia miltiorrhiza injection.Patients in the control group were treated with mannitol to decrease intracranial pres-sure;in the observation group ,nimodipine was used. For two groups’ patients before treatment ,after two weeks and three weeks of treatment ,the evaluation of neurological function and hematoma determination were carried out ,and the evaluation was made for two groups.Results No significant difference in hematoma volume between two groups was found before treat-ment.The hematoma volume of the observation group was significantly less than that of the control group after treatment of 2 weeks and 3 weeks (P<0.05);before treatment ,neurological deficit scores of two groups have no significant difference ;After two weeks and three weeks of treatment ,the neurological deficit scores of the observation group was significantly lower than that of the control group (P<0.05) ,the total effective rate of the observation group was 86.05% ,significantly better than that of the control group (65.12% , P< 0.05).Conclusion Use of nimodipine on the basis of symptomatic and supportive treatment for patients with hypertensive intracerebral hemorrhage to control intracranial pressure can significantly reduce hema-toma volume ,and promote the recovery of neurological function.
