煤工尘肺患者血清 TGF-β1和 IL-17A水平及其与肺功能关系

         

摘要

Objective To explore the relation of serum transforming growth factorβ1(TGF-β1),interleukin-17A(IL-17A)levels and lung function in patients with coal worker’s pneumoconiosis (CWP).Methods Methods The levels of serum TGF-β1 and IL-17A were determined in 118 CWP patients (CWP group),61 miners (dust exposed control group)and 53 healthy controls by sandwich ELISA;the forced vital capacity (FVC%),forced expiratory volume in one second (FEV1%),(FEV1/FVC%)and maximal mid-expiratory flow (MMEF75/25%)were determined by special lung function machine.Results Compared with healthy control group,the levels of TGF-β1 and IL-17A were signifi-cantly higher in CWP (P <0.05,P <0.01),IL-17A in CWP were significantly higher than that of dust exposed control group (P <0.01).Four parameters of lung function in CWP were lower distinctly than that of dust exposed control group(P <0.05,P <0.01).TGF-β1 levels were significantly lower in CWP with chronic obstructive pulmonary disease than that of simple CWP(P<0.01).In CWP,TGF-β1 were negatively correlated with FEV1/FVC(%)(r=-0.427,P<0.05).Conclusion The levels of serum TGF-β1 in CWP may be related to disease severity,and reflect the lung func-tion indirectly;the detection of seurm IL-17A levels may be valuable for diagnosis of CWP;TGF-β1 and IL-17A may play a role respectively in the occurrence and development of CWP.%目的:探讨血清转化生长因子β1(TGF-β1)和白细胞介素-17A(IL-17A)水平在煤工尘肺(CWP)中的变化及其与肺功能关系。方法采用双抗体夹心 ELISA 法检测118例 CWP 患者(CWP 组)、61例具有与 CWP 组相同接尘条件的健康井下接尘矿工(接尘对照组)及53例井上健康查体人员(正常对照组)血清 TGF-β1和 IL-17A 水平;用肺功能机检测用力肺活量(FVC%)、一秒钟用力呼气量(FEV1%)、用力呼气1秒率(FEV1/FVC%)、最大呼气中期流速(MMEF75/25%)4项肺功能指标。结果与正常对照组比较,CWP 组 TGF-β1和 IL-17A 水平明显升高,差异具有统计学意义(P <0.05和 P <0.01);CWP 组 IL-17A 水平明显高于接尘对照组(P <0.01),而4项肺功能指标均明显降低,差异具有统计学意义(P <0.05和 P <0.01)。CWP 并发慢性阻塞性肺疾病组 TGF-β1水平明显低于单纯CWP 组(P <0.01)。CWP 组 TGF-β1与 FEV1/FVC(%)呈负相关(r =-0.427,P <0.05)。结论CWP 患者血清TGF-β1水平可能与疾病严重程度有关,并间接反映肺功能;检测 IL-17A 水平可能对协助诊断 CWP 有一定参考价值;TGF-β1和 IL-17A 在 CWP 发生发展中可能各自发挥其作用。
