网织血小板在慢性肝病伴血小板减少症诊断中的临床价值

摘要

目的：探讨网织血小板在慢性肝病伴血小板减少症诊断中的临床价值。方法收集150例慢性肝病患者、79例特发性血小板减少性紫癜（ITP）患者和120例健康体检者，采用 XE-5000血液分析仪检测各组外周血中的网织血小板水平，并将慢性肝病患者按照血小板水平分为慢性肝病伴血小板减少症组57例，慢性肝病伴血小板正常组93例。结果慢性肝病组、ITP 组 RP、PLT 水平低于对照组，ITP 组 RP、PLT 水平低于慢性肝病组，差异有统计学意义（P ＜0．05）；慢性肝病组、ITP 组 IPF 水平高于对照组，ITP 组高于慢性肝病组，差异有统计学意义（P ＜0．05）。血小板减少症组 RP、PLT 水平低于血小板正常组，TPF 水平高于血小板正常组，差异有统计学意义（P ＜0．05）。结论通过检测网织血小板水平可有效评估慢性肝病伴血小板减少症患者骨髓巨核系统生成血小板的情况，有助于明确慢性肝病伴血小板减少症的病因。%Objective To explore clinical value of reticulated platelet in the diagnosis of chronic liver disease com-bined thrombocytopenia.Methods 150 cases with chronic liver disease,79 cases with idiopathic thrombocytopenic pur-pura (ITP)and 120 healthy volunteer were collected,whose reticulated platelet level were detected by XE-5000 blood analyzer respectively.Patients with chronic liver disease were divided into chronic liver disease with thrombocytopenia group(n=57)and chronic liver disease with normal platelet group(n=93).Results RP,PLT level in chronic liver dis-ease group and ITP group were lower than those in control group,and RP,PLT in ITP group were lower than those in chronic liver disease group,the difference was statistically significant(P <0.05).IPF in chronic liver disease group and ITP group was higher than that in control group,and IPF in ITP group was higher than that in chronic liver disease group,the difference was statistically significant(P <0.05).RP,PLT in thrombocytopenia group were lower than those in normal platelet group,TPF in thrombocytopenia group was higher than that in normal platelet group,the difference was statistically significant(P <0.05).Conclusion Detecting reticulated platelets can effectively evaluate bone marrow megakaryocytes to produce platelets in chronic liver disease combined thrombocytopenia,and it can help to confirm cause of chronic liver disease associated with thrombocytopenia.
