加长柄髋关节置换术联合钛缆治疗股骨头坏死合并同侧股骨转子间骨折的疗效

摘要

目的：观察加长柄人工全髋关节置换术联合钛缆治疗Ficat Ⅳ期股骨头缺血性坏死合并同侧股骨转子间骨折的疗效。方法将32例Ficat Ⅳ期股骨头缺血性坏死合并同侧股骨转子间骨折患者按照治疗方法分为观察组和对照组，分别采用加长柄人工全髋关节置换术联合钛缆治疗和一期股骨转子间骨折内固定治疗，二期行人工全髋关节置换术，观察两组疗效。结果观察组住院时间、术中出血量和住院费用均短于和少于对照组，P＜0.05，差异具有统计学意义。随访6～36个月后，观察组优良率94.4%略高于对照组92.8%，P＞0.05，差异不具有统计学意义。结论加长柄人工全髋关节置换术联合钛缆捆绑治疗住院时间短，创伤小，疗效好，住院费用少，并能早期下地活动，可做为治疗Ficat Ⅳ期股骨头缺血性坏死伴有同侧股骨转子间骨折的优先选择。%Objective To observe the curative effect of lengthened handle artificial total hip replacement combined with titanium cable for the treatment of ifcat stage IV femoral head avascular necrosis with ipsilateral femoral intertrochanteric fracture.Methods Selected 32 patients were divided into the observation group and the control group according to the treatment method. The two groups were treated with artificial total hip joint replacement with titanium cable and one of the femoral intertrochanteric fractures.ResultsThe hospitalization time, the amount of blood loss and hospitalization expenses in the observation group were shorter than the control group,P<0.05, had difference statistically signiifcance. Follow up 6 months to 36 months, the observation group was 94.4% slightly higher than the control group 92.8%,P>0.05, had no difference statistically signiifcance.Conclusion Lengthened handle artificial total hip replacement combined with titanium cable bundle treatment in hospital time is short, small trauma, good curative effect, less hospitalization cost and early activities, to do for the treatment of ficat IV femoral avascular necrosis is associated with femoral intertrochanteric fracture of preference.
