摘要:Objective To observe the effect of unilateral ureteral obstruction(UUO)on the ex-pression of complement 5a receptor(CSaR)in rat renal tissue,and to investigate C5aR's role during the de-velopment of renal interstitial fibrosis.Methods Thirty six male Sprague Dawley rats were randomly divid-ed into two groups(n=18),the sham-operated group and the UUO group.In the UUO group.the rat's left ureter was dissociated and obstructed.While in the sham-operated group.only rat's left ureter was disso-ciated.Six rats in each group were killed at the 5th,10th,15th day after surgery,respectively.Histologi-cal changes of renal tubular interstitium were observed with HE and Masson staining.Iminunohistochemistry Wag performed on left renal tubular interstitium for C5aR in renal tubular interstitium at each time point with ELiVisionTMplus.C5aRmRNA Was detected by RT-PCR.Results At the 5th day.there was renal tubular ectasia,interstitial cells were partially denaturated in the UUO group.At the 10th day,necrosis of intersti-tial cells and an increase of fiber in interstitial substance were found in renal tubule.While at the 15th day,there Wag agystematic thickening and shrinkage of renal tubular basement membrane,and the fibrosis was obvious(P<0.01).Injure exponent of interstitial substance and the extent of nephritic fibrosis in UUO group were much higher than that in SOR group at different time point(P<0.01).Results of immunohis-tochemistry indicated that there was only a little expression of C5aR in nephridial tissues of SOR group'srats.The expression of C5aR in the UUO group at the 5th,10th,15th day[(7.94 ±2.67)IOD,(12.39 ±2.64)IOD,(13.37±0.98)IOD]was much higher than that in SOR group[(4.50±2.32)IOD,(5.69±1.73)IOD,(4.30 ±1.45)IOD](P<0.05,P<0.01)respectively.In addition,The C5aR's expression at the 10th day was much higher than that at the 5th day(P<0.01),while the increase from the 10th day to 15th day was just a little.The results of C5aRmRNA indicated C5aR mRNA at the 10th day was much higher than that at the 5th day(P<0.01),while there was a little decrease from the 10th day to 15th day.Conclusion The expression of C5aR in renal tissue increased significantly since the 5th day after UUO,but increased slowly in the end stage.C5aR may take part in renal intersititial fibrosis at the early stage.%目的 动态检测单侧输尿管梗阻(UUO)大鼠肾组织中补体C5a受体(C5aR)的表达变化,从而探讨其在肾小管间质纤维化发展中的可能作用机制.方法 将36只雄性sD大鼠随机分为UUO组及假手术(SOR)组,每组18只,UUO组行左侧输尿管梗阻术,SOR组仅游离左侧输尿管不结扎.术后第5、10、15天分别处死每组中6只大鼠,取左肾行HE和Masson染色,并用免疫组化检测肾组织C5aR的表达,RT-PCR检测C5aR的基因表达情况.结果 UUO组第5天肾小管扩张,小管间质细胞部分变性,梗阻后第10天小管间质细胞可见坏死,间质纤维增多,第15天可见弥漫的肾小管基底膜增厚皱缩,纤维化明显,UUO组间质损伤指数与肾纤维化程度在术后不同时间点显著高于SOR组(P<0.01).免疫组化结果 显示:SOR组大鼠肾组织中仅有少量C5aR表达,UUO组术后第5、10及15天C5aR表达[(7.94±2.67)IOD,(12.39±2.64)IOD,(13.37±0.98)IOD]显著高于对应时间点的SOR组[(4.50 ±2.32)IOD,(5.69±1.73)IOD,(4.30±1.45)IOD](P<0.05,P<0.01),术后第10天明显高于第5天(P<0.01),而从第10天到第15天仅稍有增高.C5aRmRNA在UUO组从术后第5天到第10天梗阻侧肾脏C5aRmRNA表达明显增多(0.10±0.02到0.34±0.10),而从第10天到第15天表达未见明显增多,甚至有降低的趋势(第15天为0.32±0.06).结论 实验大鼠梗阻侧肾组织中C5aR的表达在早期随着梗阻时间延长而上调,晚期则增多缓慢,其基因水平表达情况同蛋白基本一致,提示C5aR可能在肾脏局部通过与循环系统中的C5a结合在肾间质纤维化早期起作用.
