Improving Patient Safety by Reducing Medication Errors. Inclusive Dates: 09/07/01-08/31/07

The overall goal of this Program was to improve patient safety by identifying the factors that predispose to medication errors, and to create a research base for the design of interventions to reduce the frequency of medication errors. A research base was developed for designing system solutions that are amenable to intervention, including technical, cultural, and human factors, in order to reduce medication errors and their associated huge human and economic costs. The Program combined investigators in multiple areas to address the theme Improving Patient Safety through Reduction of Errors in the Medication Use Process. The Program was composed of four Projects and four Cores, based at the University of Pennsylvania and linked to the government of the State of Pennsylvania and to the network of Centers for Education and Research in Therapeutics. Each of the four Cores served the four Projects in such a way as to maximize quality and efficiency simultaneously.
