When something goes wrong in hospital, the first step is to care for the patient and contain the situation, hopefully bringing about an acceptable, if not good, end. Sometimes this doesn't happen, and the cycle of blame starts. Not only can the blame come from the patient, but from the families and sometimes support organizations. In most cases, the patient's medical record is thorough enough to give those groups the information they need to make a decision about their next steps.
