A 50-year-old woman received conservative management for a thalamic intracerebral hemorrhage. The patient underwent rehabilitation beginning 2 wks after onset and showed profound limitation of movement of the right eye (fixed in the midline without movement in any direction) with mydriasis and diplopia. Limitation of eye movements recovered rapidly, however, and the patient was able to move her right eye in all directions with full range of motion at 6 wks after the onset. The diplopia had almost disappeared at 8 mos after the onset. Brain magnetic resonance images taken at 4 days after the onset showed a thalamic hemorrhage and severe perilesional edema in the right midbrain and posterior limb of the internal capsule (Fig. 1A).
