Dr. Zafar pioneered the subspecialty of pediatric and adolescent gynecology in Pakistan while serving as assistant professor in pediatric and adolescent gynecology at Children's Hospital and Institute of Child Health, Lahore, Pakistan. She is the Founder and President of The Girls and Women Health Initiative (GWHI), a not-for-profit charitable venture dedicated to the health and well-being of girls and women, especially the poor and underprivileged. Dr. Zafar has pioneered, through television, radio, and talks, open discussion of gynecology and reproductive health, previously widely considered a shameful topic. Through an awareness campaign, featuring the slogan "Wellness with Dignity," Dr. Zafar hopes to sensitize the Pakistani society to end gender-based discrimination.
