PROBLEM TO BE SOLVED: To provide a medical interview sheet creation system for reducing the burden of a patient in creating a medical interview sheet, a mobile information terminal, a medical interview sheet creation method, a medical interview sheet creation program, and a medical interview sheet management server.SOLUTION: A medical interview sheet creation system includes: a memory 11 for holding individual data 11a identifying an individual; a microphone 13 that generates voice data in a foreign language on receipt of a voice in the foreign language for explaining a symptom; a voice recognition section 11d which converts the voice data to text data in the foreign language; a translation server 20 which translates the text data in the foreign language into text data in Japanese; a medical interview sheet creation program 11c that synthesizes the individual data 11a and the text data in Japanese with a template of a medical interview sheet, to create medical interview sheet data; and a touch panel 12 for displaying the medical interview sheet data. The medical interview sheet creation program 11c synthesizes image data of an affected area captured by a camera 14 with the template, to create the medical interview sheet data.SELECTED DRAWING: Figure 2
