新生儿血清25-羟基维生素D水平检测及临床意义

摘要

目的：评价血清25-羟基维生素D[25（OH）D]水平在新生儿不同疾病的诊断价值。方法依据不同性别、年龄、季节、疾病对797例新生儿进行分组，采用电化学发光免疫分析法，分别测定各组新生儿血清中[25（OH）D]含量，并统计分析各组间[25（OH） D]水平差异。结果男性新生儿血清[25（OH）D]水平为（14.91±7.55）（14.20~15.61）ng/ml，女性为（15.51±7.43）（14.73~16.29） ng/ml；不同性别新生儿[25（OH）D]水平比较，差异无统计学意义（t=1.483，P=0.138）。不同日龄新生儿血清[25（OH）D]水平比较，差异无统计学意义（F=1.875，P=0.132）。不同季节新生儿血清[25（OH）D]水平以秋季最高，夏季最低，秋冬季明显高于春夏季，差异有统计学意义（F=6.262，<0.001）。不同疾病组新生儿血清[25（OH）D]水平比较，差异有统计学意义（F=3.112，P=0.002）。其中，以败血症、上呼吸道感染、黄疸和早产儿组[25（OH）D]水平较低；而吸入综合征、糖尿病母亲的婴儿综合征、窒息和肺炎组新生儿[25（OH） D]水平较高，差异有统计学意义（P<0.01）。797例住院新生儿维生素D营养状况为：维生素D适宜188例，不足158例，缺乏426例，严重缺乏25例，其构成比分别为23.59%、19.82%、53.45%；和3.14%。不同疾病组间新生儿维生素D营养状况比较，差异有统计学意义（χ2=25.408， P=0.001）。结论不同性别、不同年龄的新生儿血清[25（OH）D]水平不同，差异无统计学意义。不同季节、不同疾病新生儿血清[25（OH）D]水平及营养状况不同。差异有统计学意义。%ObjectiveTo evaluate the diagnostic value of the serum level of 25-hydroxyvitamin D3[25(OH)D3]in newborn.Method797 newborn were selected and divided to different groups by gender,age,season,and disease they suffering. the serum level of 25-hydroxyvitamin D3[25 (OH)D3] was detected by electro-chemiluminescence immunoassay.ResultThe serum level of 25-hydroxyvitamin D3 in male and female newborn were 14.91±7.55(14.20~15.61)ng/ml and 15.51±7.43(14.73~16.29)ng/ml respectively,there is no statistical significance between different gender(t=1.483,P=0.138)and age(F=1.875,P=0.132). There was statistical significance(F=6.262,<0.001)among different seasons,with a highest value in autumn and a lowest in the summer. There is statistical significance(F=3.112,P=0.002)in newborns with different diseases. with lower level of 25-hydroxyvitamin D3 in newborn with upper respiratory tract infection,sepsis,jaundice,and preterm newborn and a higher level in newborn with aspiration syndrome,diabetic mothers,asphyxia and neonatal pneumonia. There was statistical significance(χ2=25.408,P=0.001) among different diseases.Conclusions There was no statistical significance between different gender and age. There is statistical significance of serum level of 25-hydroxyvitamin D3 in newborn with different diseases and seasons.
