七氟醚联合地佐辛复合麻醉用于小儿腺样体肥大切除术中镇痛的应用研究

摘要

To investigate the analgesic effect of sevoflurane combined with dezocine inhalation anesthesia in pediatric adenoid hypertrophy resection.Methods:84 cases of adenoid hypertrophy resection were divided into con-trol group and observation group according to random number table method,42 cases in each group.The control group was treated with intravenous analgesic,and the observation group was treated with sevoflurane combined with dezocine inhalation anesthesia.Mean arterial pressure(MAP),heart rate(HR),visual analogue scale(VAS)score,Ramsy sedation score and postoperative adverse reactions were compared between the two groups.Results:There were no significant differences in MAP,HR,VAS and Ramsay scores between the two groups before anes-thesia(P>0.05).The levels of MAP and HR in the two groups were lower than those in the control group,the difference was statistically significant(P<0.05).The incidence of postoperative adverse reactions was 9.52(4/42)in the observation group,which was lower than that of the control group(30.95％,13/42)(P<0.05).Conclu-sion:In the pediatric adenoid hypertrophic resection,the implementation of sevoflurane combined with dezocine in-halation anesthesia has a good analgesic effect,can effectively reduce the proportion of postoperative adverse reac-tions,stable hemodynamics,and the respiratory rate Obviously,more secure and reliable.%目的:探讨七氟醚联合地佐辛复合麻醉在小儿腺样体肥大切除术中的镇痛效果.方法:腺样体肥大切除术患儿84例,根据随机数表法分为对照组、观察组,各42例.对照组采用静脉注射地佐辛镇痛,观察组应用七氟醚联合地佐辛复合麻醉的方式镇痛.比较两组患者血流动力学[(平均动脉压(MAP)、心率(HR)]、视觉模拟评分法(VAS)评分、Ramsy 镇静评分及术后不良反应.结果:麻醉前两组患儿MAP、HR指标,VAS及Ramsay评分差异无统计学意义(P>0.05);麻醉后对照组MAP、HR高于麻醉前(P<0.05),观察组MAP、HR与麻醉前比较差异无统计学意义(P>0.05);麻醉后对照组VAS评分、Ramsay评分高于观察组(P<0.05).观察组术后不良反应发生率9.52(4/42),低于对照组的30.95％(13/42)(P<0.05).结论:在小儿腺样体肥大切除术中实施七氟醚联合地佐辛复合麻醉具有较好的镇痛效果,能够有效降低术后不良反应发生比例,稳定血流动力学,且对呼吸频率无明显影响,更加安全可靠.
