目的 评价糖皮质激素治疗成人Still病(AOSD)的有效性.方法 45例诊断为AOSD的患者均接受糖皮质激素治疗,观察激素治疗前后发热、皮疹、关节痛等临床表现及白细胞、血沉、C反应蛋白、血清铁蛋白、谷丙转氨酶等化验指标均值的变化.结果 糖皮质激素治疗的有效率为71.1%,平均有效剂量为(72.8±46.5)mg/d.激素治疗后大多数患者发热、皮疹、关节痛症状明显改善,白细胞、血沉、C反应蛋白、铁蛋白等化验指标较治疗前明显下降,差异有统计学意义(P<0.01),谷丙转氨酶较治疗前下降,但差异无统计学意义(P>0.05).结论 糖皮质激素治疗AOSD效果明显,是目前治疗AOSD的首选药物.%Objective To evaluate the efficacy of glucocorticoids on adult-onset still's disease. Methods 45 cases with AOSD were treated with glucocorticoids, to observe the variation of clinical manifestations such as fever, rash, arthralgia and the mean value of laboratory index such as WBC, ESR, CRP, SF, ALT between before and after treatment. Results The efficacy of glucocorticoids treatment was 71. 1% ,the mean effective dose was( 72. 8 ±46. 5 ) mg/d. The symptoms of fever,rash, arthralgia had obvious improvement in the majority of patients after treatment, the laboratory indexes such as WBC,ESR,CRP,SF were obviously decreased compared with before treatment,the difference was statistically significant(P < 0. 01 ), ALT decreased compared with before treatment, the difference was not statistically significant(P >0. 05 ). Conclusion The glucocorticoids treatment is effective,so it is the first choice for the treatment of AOSD currently.
