Objective To observe the adverse effects of the thrombolysis treatment with alteplase for the patients with moderate to high pulmonary embolism and analyze the nursing intervention to prevent and treat the complications. Methods Thirty-nine patients of pulmonary embolism with moderate to high risks who received alteplase for thrombolysis were collected. The adverse effects during and after the thrombolysis were recorded. The nursing measures were given. Results Totally 38 patients completed the thrombolysis treatment with no severe adverse effect. There were 3 cases of positive occult stools with no tarry stools or bloody stool,2 cases of local hematoma. With supportive treatment, all the adverse effects were corrected. Conclusion Careful observation and monitoring, cooperation of the doctors and nurses and the prevention of the complications such as bleeding is critical for the successful thrombolysis with 50 mg alteplase intravenous injection within 2 h.%目的 观察肺栓塞中高危患者使用阿替普酶溶栓过程中出现的不良反应,分析其相关护理干预,预防并发症的出现.方法 收集我院呼吸内科收治的肺栓塞中高危且接受阿替普酶溶栓的患者39例,观察在溶栓治疗中出现的不良反应,并给予相应的护理干预.结果 经积极干预,完成溶栓治疗共38例,未出现严重不良反应.2例出现便潜血阳性,但无柏油样便及血便;3例出现动脉采血局部血肿;1例出现低血压.经对症处理后均好转.结论 应用50 mg阿替普酶2 h缓慢推注治疗溶栓过程中,严密的观察与监护、医护人员的密切配合及防治出血等并发症是溶栓成功的关键.
