输尿管皮肤造口患者单J管更换就诊延迟的质性研究

摘要

目的 深入了解输尿管皮肤造口患者对单J管更换就诊延迟的认知及影响因素,为采取更有效的干预提供依据.方法 运用质性研究中的现象学研究方法,分析14例输尿管皮肤造口患者单J管更换就诊延迟的真实感受.结果 患者单J管更换就诊延迟主要表现为焦虑、侥幸和麻木3种心理;经济保障不足、路途遥远、家庭支持不足和相关知识缺乏是影响其就诊延迟的原因.结论 输尿管皮肤造口患者术后门诊单J管定期更换依从性差,既有主观上的认知缺陷,也有客观上的就医障碍.期待多部门共同合作,增强患者门诊定期换管的依从性,改善其生活质量.%Objective To understand the cognition and influencing factors of delayed replacement of single J tube among patients after ureterocutaneostomy,and provide reference for effective intervention.Methods The phenomenological methodology was used to analyze the true feelings of delayed replacement of single J tube among 14 patients after ureterocutaneostomy.Results Patients' delayed replacement of single J tube mainly manifested as anxiety,luck and numbness;factors influencing patients' treatment delay were shortage of economic security,far distance,insufficient family support and lack of related knowledge.Conclusion The compliance of single J tube periodical replacement in outpatient department among patients after ureterocutaneostomy is poor,which results from subjective cognitive deficits and objective medical service barriers.It is expected that health departments cooperate to strengthen patients' compliance of periodical replacement of single J tube in outpatient department,then to improve their quality of life.
