青少年精索静脉曲张手术效果研究

摘要

Objective:To evaluate the testicular volume and fertility potential in men after varicocele ligation to my conducted in adolescence due to varicocele and concomitant testicular hypotrophy .Methods:40 cases were primary vari‐cocele patients with testicular atrophy in adolescents ,conducted retroperitoneal varicocele ligation amputation treat‐ment ,34 patients were status quo analysis ,evaluation testicular size and sperm the severity of the varicocele .Results:An increase in left testicular volume when compared with the contralateral testis was found in 13 (81 .25% ) young men with clinical grade 2 varicocele (P=0 .01) and in 15 (83 .33% ) subjects with grade 3 abnormality(P=0 .02) .An in‐crease in left testicular volume was found in 23(85 .19% ) of 27 patients with unilateral varicocele and in 5 (71 .43% ) of 7 patients subjects operated on for bilateral disease .A left testicular volume increase was comparable independent of the use of uni‐or bilateral repair .29 (85 .29% ) of our 34 patients had normal zoospermia .Conclusion:Varicocele repair resulted in a significant increase of hypotrophy testicular volume in 82 .35% of our subjects .%目的：对精索静脉曲张合并睾丸萎缩的青少年患者进行精索静脉高位结扎术，随访并评估患者睾丸体积和生育的潜力。方法：选取40例均为原发性精索静脉曲张伴有睾丸萎缩的青少年患者，进行腹膜后精索静脉高位结扎＋离断术治疗，对其中34例患者进行现状分析，评估睾丸大小和精索静脉曲张的严重程度。结果：双侧比较，临床2级患者13例（81．25％）左睾丸体积增加（ P＝0．01），临床3级患者15例（83．33％）左睾丸体积增加（ P＝0．02）。27例单侧精索静脉曲张中，23例（85．19％）左睾丸体积增加。7例双侧患者，5例（71．43％）左睾丸体积增加。术后随访2～10年，34例患者29例（85．29％）精子检查正常。结论：精索静脉曲张手术使82．35％的睾丸萎缩患者的睾丸体积明显增大。手术对青少年精索静脉曲张治疗有很大的益处。
