CEA、CA15-3检测对良恶性胸腔积液的诊断价值

摘要

目的 探讨胸腔积液患者CEA、CA15-3表达水平及其对良恶性胸腔积液的诊断价值.方法 应用ARCHITECT i2000SR免疫分析检测仪对45例恶性胸腔积液患者进行血清和胸腔积液CEA、CA15-3测定,并与32例良性胸腔积液患者作比较.结果 恶性胸腔积液组血清和胸腔积液表达均显著高于良性胸腔积液组的表达(P<0.05);恶性胸腔积液组CEA和CA15-3的胸腔积液/血清比值显著高于良性组(P<0.05);胸腔积液CEA、CA15-3的表达同血清中的表达比较具有显著性差异(P<0.05).单项肿瘤标志物检测胸腔积液CEA检测的准确度最高,两项指标联合检测胸腔积液检测的准确度高于血清检测的准确度(P<0.05).结论 检测胸腔积液患者CEA、CA15-3表达水平有助于积液性质的良恶性鉴别.胸腔积液CEA、CA15-3的检测对恶性胸腔积液的诊断较血清学检测更有价值.多肿瘤标志物联合检测能提高恶性胸腔积液诊断的准确度.%Objective To evaluate CEA, CA15 -3 expression in the patients with pleural effusions and to evaluate the diagnostic value far differentiating malignant and benign pleural effusions.Methods ARCHffECT i2000SR immunoassay was applied lo detect serum and pleural effusians CEA, CA15 -3 concentration of the patients with malignant pleural effusions(45 cases) as well as 32 cases patients with henign pleural effusions.Results CEA, CA15 -3 expression in serum and pleural effusions was aignificantly higher in the patients with malignant pleural effusions than that with benign pleural effusions( P ＜ 0.05 ) .The ratio of CEA , CA15 - 3 in pleural effusions to serum was significantly higher in the patienta with malignant pleural effUBions than those with benign pleural effusions( P ＜ 0.05 ) .The CEA , CA15 - 3 expression in pleural effusions was significantly higher than that in serum( P ＜ 0.05 ) .To detect CEA expression in pleural effusions had higher accuracy than the others single tumor mark detection.The accuracy of CEA and CA15 -3 combined measurement in pleural effusions was higher than serum detection.Conclusion To detect CEA, CA15 -3 expression in the patients with pleural effusions was helpful for differentiating malignant and benign pleural effuaions.Pleural effusion CEA, CA15 - 3 expression is more valuable on malignant pleural effusions diagnosis than serological test.Multiple tumor markers combined measurement can improve the diagnosis accuracy of malignant pleural effusion.
