血浆同型半胱氨酸与冠状动脉病变程度的相关性研究

             

摘要

目的 探讨血浆同型半胱氨酸(Hcy)与冠状动脉病变程度的相关性.方法 380例因胸痛患者行冠状动脉造影并测定血浆Hcy水平,采用Gensini评分系统对患者的冠状动脉病变程度进行评估.分析Hcy水平与Gensini评分的相关性.结果 380例中270例确诊冠心病,其中稳定性心绞痛(SAP)140例,非ST段抬高型急性冠脉综合征(NSTE-ACS)组130例;非冠心病者110例为对照组.根据Hcy水平分组,>10μmol/L265例(高Hcy组),≤10μmol/L115例(非高Hcy组).冠心病组Hcy水平显著高于对照组(P<0.01),高Hcy组的Gensini评分显著高于非高Hcy组(P<0.01),三支及双支病变组Hcy水平显著高于单支病变组(P<0.01).Spearman秩相关分析表明Hcy水平与Gensini积分呈正相关.结论 血浆Hcy水平与冠状动脉病变程度相关.%Objective To explore the relationship between the plasma level of homocysteine (Hcy) and the severity of coronary artery disease (CAD). Methods 380 patients underwent coronary angiography and measurement of plasma Hcy for chest pain. The severity of coronary artery lesions was evaluated based on Gensini score system. The correlation between plasma Hcy and Gensini score was analyzed. Results Of 380 patients, 270 cases was diagnosed as CAD (140 stable angina pectoris and 130 non-ST-segment elevation myocardial infarction) and 110 non-CAD. The level of Hcy increased (>10μmol/L) in 265 cases and was normal in 115 cases. The level of Hcy was significantly higher in CAD patients than in non-CAD patients (P<0.01). Gensini score was significantly higher in patients with Hcy increased than with normal Hcy, and significantly higher in CAD patients with two-or three-vessel lesions than with only one-vessel lesion (all P<0.01). Spearsman correlation analysis showed that Hcy level was positively correlated with the Gensini score. Conclusion Plasma Hcy levels is related to the severity of coronary artery lesions.
