小儿肺炎血清CK、CK-MB、cTnT及ECG变化在治疗及预后中的应用研究

         

摘要

目的 研究小儿肺炎患儿血清CK、CK-MB、cTnT水平及ECG变化特征在疾病治疗及预后评价中的应用.方法 分别选取2014年5月-2015年5月我院收治肺炎感染50例作为治疗组及健康体检儿童48例作为对照组,依照患儿的严重程度将治疗组分为轻症组和重症组;检测所有受试者心肌酶谱、肌钙蛋白T及心电图情况并进行比较.结果 轻症组及重症组CK、CK-MB、LDH、AST、cTnT水平均显著高于对照组(P<0.05);治疗后,对照组与重症组CK-MB及cTnT存显著性差异(P<0.05);与治疗前相比,治疗后重症组CK、CK-MB及cTnT水平均显著降低(P<0.05);治疗前轻症组及重症组患儿心电图异常率与对照组存显著性差异(P<0.05);轻症组与重症组ECG差异存统计学差异(P<0.05);治疗前,轻症组及重症组CK、CK-MB、cTnT水平均显著高于对照组(P<0.05),且重症组也显著高于轻症组(P<0.05).结论 对患者CK、CK-MB及cTnT水平进行检测并与ECG数据同时考虑,有利于小儿肺炎的诊断及预后判断.%Objective To explore the application of serum CK, CK-MB, cTnT levels and ECG changes in the treatment and prognosis of children with pneumonia. Methods 50 children with pneumonia infection were selected from May 2014 to 2015 as the treatment group, and another 48 healthy children were taken as the control group. In accordance with the severity of children in the treatment group, they were divided into the mild group and the severe group. Their myocardial enzymes, cardiac troponin T ( cTnT) and ECG were compared. Results The levels of CK,CK-MB, LDH, AST and cTnT were significantly higher in the treatment group than in the control group (P<0. 05). After treatment, the levels of CK-MB and cTnT were significantly different between the control group and the severe group (P<0. 05), and the levels of CK, CK-MB and cTnT decreased significantly in the severe group (P<0. 05). There was significant difference in abnormal electrocardiogram ( ECG) rate between the treatment group and the con-trol group before treatment and between the mild group and the severe group (P<0. 05). The levels of CK, CK-MB and cTnT were significantly higher in the treatment group than in the control group (P<0. 05), and it was also obvi-ously higher in the severe group than in the mild group (P<0. 05). Conclusion The detection of CK, CK-MB and cTnT levels and the comparison with ECG data are conducive to the diagnosis and prognosis of pneumonia in children.
