四川地区糖尿病、糖调节受损人群胃泌素、胃蛋白酶原水平及幽门螺杆菌感染情况分析

         

摘要

Objective To discuss the difference of gastrin-17 (G-17),pepsinogen Ⅰ (PG Ⅰ),pepsinogen Ⅱ (PGⅡ) levels and Helicobacter pylori(Hp) infection conditions between diabetes mellitus (DM),impaired glucose regulation (IGR) and normal people in Sichuan district.Methods A total of 2 191 individuals who received health check-up in health physical examination center of Sichuan Provincial People's Hospital during January 2015 to August 2016 were selected and divided into DM group(839 cases),IGR group(352 cases) and control group(1 000 cases) according to the blood glucose levels.G-17,PG Ⅰ and PG Ⅱ were measured by enzyme linked immunosorbent assay(ELISA) and Hp infection conditions were tested by 13C breath test.Results The levels of G-17,PG Ⅰ and PG Ⅱ in the DM,IGR and control group decreased successively(P < 0.01).With the increase of glycosylated hemoglobin (HbAlc) level,the levels of G-17,PG Ⅰ and PG Ⅱ in the subjects showed an upward trend,and the differences among the 4 groups with the different HbAlc levels were statistically significant(P < 0.01).The rates of Hp infection decreased successively in DM,IGR and control group (P < 0.01),and in the same group,the levels of G-17,PG Ⅰ and PG Ⅱ in Hp(+) patients were higher than those in Hp(-) patients (P < 0.05).Conclusion G-17,PG Ⅰ and PG Ⅱ levels in DM and IGR are higher than normal people,and the increase is more obvious in Hp positive patients.%目的 探讨四川地区糖尿病、糖调节受损及正常人群胃泌素-17(G-17)、胃蛋白酶原Ⅰ(PGⅠ)、胃蛋白酶原Ⅱ(PGⅡ)水平及幽门螺杆菌(Hp)感染情况的差异.方法 选取2015年1月~2016年8月在四川省人民医院健康体检中心体检者共2 191例,根据血糖水平分为糖尿病组(839例)、糖调节受损组(352例)和对照组(1000例),采用酶联免疫吸附试验(ELISA)测定G-17、PG Ⅰ及PGⅡ水平,并通过13C呼气试验了解其Hp感染情况.结果 糖尿病组、糖调节受损组及对照组受试者的G-17、PG Ⅰ、PGⅡ水平均依次降低,3组间比较差异有统计学意义(P<0.01).随着糖化血红蛋白(HbA1c)水平升高,受试者的G-17、PG Ⅰ、PGⅡ水平均呈上升趋势,4组不同HbAlc水平受试者比较差异有统计学意义(P<0.01).糖尿病组、糖调节受损组和对照组的Hp感染率依次降低(P<0.01),且在同一组中,Hp(+)者G-17、PG Ⅰ、PGⅡ水平也高于Hp(-)者(P<0.05).结论 糖尿病及糖调节受损者G-17、PGⅠ、PGⅡ等水平均有所升高,Hp感染阳性者升高更明显.
