CRP、IL-8及IL-17水平在COPD稳定期患者中的临床意义及其与BODE指数的相关性

摘要

目的：探讨血清C反应蛋白（CRP）、白细胞介素-8（IL-8）及白细胞介素-17（IL-17）水平的检测在慢性阻塞性肺疾病（ COPD）稳定期患者中的临床意义及其与BODE指数的相关性。方法选取2013年2月至2014年11月接受治疗及随访的38例COPD患者（观察组）的临床资料进行详细分析，另外选取同期的38例健康体检者为对照组。比较两组的基线资料、血清CRP、IL-8、IL-17的水平等。观察COPD患者血清IL-8、IL-17及CRP水平与BODE指数及其构成变量的相关性分析。结果两组基线资料的组间差异无统计学意义（ P >0.05）。两组血清IL-8、IL-17及CRP水平的组间差异均有统计学意义（ P <0.01）。观察组患者血清CRP、IL-8及IL-17水平与与BODE指数均存在明显的正相关关系，其中与6 min步行距离（6MWD）、第1秒用力呼气容积所占的预计值的百分比（ FEV1%pred）均呈显著的负相关关系，而与改良呼吸困难量表（ MMRC）有明显的正相关关系；此外IL-8水平与BMI也存在负相关性。结论血清CRP、IL-8及IL-17水平在COPD稳定期患者中呈高表达状态，其与BODE指数均呈显著的正相关关系，这三种因子的检测能有效预测此类患者的病情及预后。%Objective To study the clinical significance of the detection of the levels of C reactive protein( CRP),Interleukin-8( IL-8),Interleukin-17(IL-17)the stable COPD patients and the correlation with the BODE index. Methods The Clinical data of 38 cases of pa-tients with COPD( observe group)received from February 2013 to November 2014 were retrospectivelyanalyzed. 38 cases of health checkup persons at the same time were chosen as the control group in this research. The general information,the levels of CRP,IL-8,IL-17 and so on were compared. The levels of IL-8,IL-17,CRP in patients with COPD was observed,and correlation between variables BODE index and its compo-nents was analyzed. Results The general information in two groups had no statistical difference. The levels of CRP,IL-8,IL-17 in two groups had statistical differences. The levels of CRP,IL-8,IL-17 of patients in observe group all had positive correlation with the BODE index. The 6MWD and FEV1%pred all had obvious positive correlation with the levels of CRP,IL-8,IL-17,and the MMAC had negative correlation with the levels of CRP,IL-8,IL-17. The level of IL-8 had negative correlation with BMI. Conclusion The levels of CRP,IL-8,IL-17 in pa-tients with stable COPD are at the state of high expression. The detection of the above inflammatory cytokines can effectively predict the condition and prognosis of patients with COPD.
