血浆胱蛋白酶抑制剂C在评估肾小球滤过率中的应用

         

摘要

Objective: To explore the value of cystatin C in assessing glomerular filtration rate. Methods: One hundred and nine patients were devided into 4 groups according to the level of endogenous creatinine clearance rate(Ccr) ,which included normal renal function group ( Ccr ≥ 80 ml/min, n = 33 ) , renal insufficiency compensation group (50 ml/min ≥ Ccr < 80 ml/min, n = 20 ) , renal insufficiency decompensation group (20 ml/min ^ Ccr < 50 ml/min, n = 27 ) and renal failure group ( Ccr < 20 ml/min, n = 29 ) . Sixty-nine healthy people were set as control group. The levels of cystatin C, Scr and urea nitrogen in blood plasma of all group were determined, which were statistically analyzed. Results: Compared with control group, there were significant differences in the cystatin C levels of other groups ( P < 0. 01 ). The levels of Cystatin C increased along with the Ccr levels decline. There were good relevant relationships between Ccr and Scr ( P < 0. 01). Conclusions: Blood plasma cystatin C is a sensitive indicator for evaluating the glomerular filtration function, which can replace the role of Ccr.%目的:探讨胱蛋白酶抑制剂C(cystatin C)在评估肾小球滤过率中的价值.方法:将109例患者按内生肌酐清除率(Ccr)水平分4组:肾功能正常组(Ccr≥80 ml/min)33例,肾功能不全代偿组(50 ml/min≤Ccr<80 ml/min)20例,肾功能失代偿组(20 ml/min≤Ccr <50 ml/min)27例,肾功能衰竭组(Ccr <20 ml/min)29例;另设正常对照组69名.分别检测观察对象血浆cystatin C、血肌酐(Scr)、尿素氮水平,计算其Ccr,并将结果进行统计学分析.结果:其他肾功能组cystatin C结果与健康对照组比较,差异均有统计学意义(P<0.01),并随着Ccr的下降而上升,且与Ccr和Scr均具有较好的相关关系(P<0.01).结论:血浆cystatin C检测是判断肾小球滤过功能的敏感指标,其结果可替代内生Ccr.
