目的：探讨标准大骨瓣减压联合顺序硬脑膜剪开术治疗特重型颅脑损伤的疗效。方法取2013年6月～2014年6月48例特重型颅脑损伤病例并均分为对照组与观察组各24例，两组均行标准大骨瓣减压术，对照组行一次硬脑膜剪开术处理，观察组则采取顺序硬脑膜剪开术处理行逐次彻底减压。对比两组疗效。结果观察组患者术中瞳孔恢复率较对照组更高、急性脑膨出率更低，组间差异显著而具有统计学意义（ P＜0．05）；与对照组相比，观察组患者术后6月预后良好率更高，而死亡率更低（ P＜0．05）。结论标准大骨瓣减压联合顺序硬脑膜剪开术用于特重型颅脑损伤效果佳，患者预后良好，应在临床治疗中推广应用。%Objective To study the standard big bone flap decompression for the treatment of open a joint order dura track, the curative effect of traumatic brain injury.Methods Apply during June 2013 to June 2014, 48 cases of track, craniocerebral injury cases and each 24 cases were divided into control group and observation group, two groups of line standard big bone flap decompression, the control line cut open a dura mater and surgery treatment group are taking order dura cut processing line successive thoroughly decompression.Compare two groups of curative effect.Results Observa-tion group of patients with intraoperative pupil recovery rate higher than the control group, the rate of acute encephalocele are lower, significant difference between groups with statistical significance ( P<0.05);In June compared with control group, the observation group of patients with postoperative prognosis is good rate is higher, and lower mortality ( P<0.05).Conclusions Standard big bone flap decompression combined order cut open dura technique is used to track the effect of traumatic brain injury, patients prognosis is good, should be applied in clinical treatment.
