吗替麦考酚酯对儿童紫癜性肾炎血浆FDP和Fib、血清白蛋白和24h尿蛋白的影响研究

摘要

Objective To investigate the effect of mycophenolate mofetil on plasma FDP,Fib,serum albumin,24 h urine protein levels in children with purpura nephritis and its curative effect.Methods 68 children with purpura nephritis were randomly divided into control group (n=34) and observation group (n=34).Control group was treated with conventional therapy,and observation group was treated with mycophenolate mofetil on the basis of control group.The treatment course of two groups was 6 months.Results Plasma FDP,Fib levels significantly decreased after treatment in both two groups (P＜0.05);plasma FDP,Fib levels in observation group after treatment were lower than those in control group (P＜0.05).Serum albumin level increased after treatment in both two groups,24 h urine protein level decreased (P＜0.05);serum albumin level in observation group after treatment was higher than that in control group,and 24 h urine protein level was lower than that in control group (P＜0.05).The total effective rate in observation group was higher than that in control group (94.12％ vs.70.59％,P＜0.05).There were no obvious adverse reactions in both two groups.Conclusion The clinical effect of mycophenolate mofetil in the treatment of purpura nephritis in children is significant,which can significantly decrease plasma FDP,Fib,serum 24 h urine protein levels and increase serum albumin level,thus relieve symptoms in children.%目的 探讨吗替麦考酚酯对儿童紫癜性肾炎血浆FDP和Fib、血清白蛋白和24 h尿蛋白水平的影响及其疗效.方法 儿童紫癜性肾炎患者68例随机分为对照组与观察组各34例.对照组以常规治疗,观察组结合吗替麦考酚酯治疗.两组疗程为6个月.结果 两组患儿治疗后血浆FDP、Fib水平明显降低(P＜0.05),观察组患儿治疗后血浆FDP、Fib水平低于对照组(P＜0.05);两组患儿治疗后血清白蛋白增加,24 h尿蛋白下降(P＜0.05),观察组患儿治疗后血清白蛋白高于对照组,24 h尿蛋白低于对照组(P＜0.05);观察组总有效率(94.12％)高于对照组(70.59％)(P＜0.05);两组患儿均未出现明显的不良反应.结论 吗替麦考酚酯治疗儿童紫癜性肾炎疗效显著,且可明显降低患儿血浆FDP和Fib及血清24 h尿蛋白水平,增加白蛋白,从而减轻患儿症状、体征.
