黏膜免疫介导的IgA肾病发病机制及治疗

摘要

IgA肾病 (Immunoglobulin A Nephropathy, IgAN) 与黏膜免疫系统 (Mucosal immune system, MIS) 的异常激活及慢性炎症状态相关, 其中以扁桃体、肠黏膜免疫系统最为重要.黏膜免疫系统通过黏膜表面微生物群、食物成分、异常免疫应答等介导IgAN发病.目前与之相应的治疗研究热点:扁桃体切除术尚存争议、肠黏膜靶向激素治疗需进一步确证, 而调节肠道菌群、控制食物种类、综合治疗等可作为未来研究方向.%IgA Nephropathy (IgAN) is related with the Abnormal activation and chronic inflammatory state of Mucosal immune system (MIS), and the immune system of tonsil and intestinal mucosa is the most important. MIS leads to IgAN through mucosal sur-face microflora, food composition and abnormal immune response, etc. At present, the corresponding treatment research hotspotsare as follows:tonsillectomy remains controversial, and hormonotherapy targeting intestinal mucosal immunity needs further confir-mation.And regulating the intestinal flora, controlling the food species and comprehensive treatment can be the future research direction.
