FIESTA和FRFSE FATSAT T2WI序列诊断主动脉夹层动脉瘤效果比较

         

摘要

目的 评价磁共振非对比增强稳态采集快速成像序列(FIESTA)和快速恢复快速自旋回波T2加权抑脂成像序列(FRFSE FATSAT T2WI)诊断主动脉夹层动脉瘤(AD)的优劣性及其临床价值.方法 成都市第五人民医院放射科2013年6月至2014年1月期间临床确诊为AD患者24例,应用1.5T磁共振FIESTA序列和FRFSE FATSAT T2WI序列扫描,分析两种序列对AD内膜及其破口的显示情况,评价AD真、假腔内信号变化及血栓显示情况.结果 24例AD患者,两种序列显示内膜片及破口差异均无统计学意义(P>0.05);FIESTA序列显示假腔内血栓优于FRFSE FATSAT T2WI序列,差异有统计学意义(P<0.05);FIESTA序列显示真腔附壁血栓与FRFSE FATSAT T2WI序列之间,差异无统计学意义(P>0.05);通过11例假腔内无血栓AD真、假腔内信号对比分析可知,FRFSE FATSAT T2WI序列在判断AD真假腔时可提供较大的帮助.结论 FIESTA和FRFSE FATSAT T2WI序列在评价AD时各有优缺点且优势互补,联合应用两种序列可对AD进行全面评价.%Objective To evaluate the fast imaging employing steady-state acquisition (FIESTA) and the fast relaxation fast spin echo with fat saturation T2 weighted imaging (FRFSE FATSAT T2WI) sequences'superiority in the diagnosis of aortic dissecting aneurysm (AD) and their clinical value. Methods A total of 24 patients, who were clini-cally diagnosed with AD and admitted to Department of Radiology the Fifth People's Hospital of Chengdu City from June 2013 to January 2014, were selected as the research subjects. All the research subjects were scanned with 1.5T mag-netic resonance FIESTA sequence and FRFSE FATSAT T2WI sequence to evaluate AD endometrium and its rupture. The changes of AD and intramuscular changes were also observed by the two sequences. Results There was no signifi-cant difference between two kinds of sequence in the intimal flap and crevasse of 24 cases (P>0.05). FIESTA sequence showed the false thrombus was significantly superior to FRFSE FATSAT T2WI sequence (P<0.05). There was no signifi-cant difference between FIESTA sequence and FRFSE FATSAT T2WI sequence in showing the true lumen mural throm-bus (P>0.05). FRFSE FATSAT T2WI sequence can provide great help in judging the true and false thrombus of AD through comparison analysis of the true and false cavity signal of 11 cases of AD with no intraluminal thrombus in the false cavity. Conclusion FIESTA and FRFSE FATSAT T2WI sequences have their advantages and disadvantages in evaluating AD, and the two sequences can be used in combination to evaluate AD.
