骨水泥在椎弓根钉内固定治疗骨质疏松性椎体骨折中的应用

         

摘要

Objective To evaluate the relationship between the dose of PMMA bone cement and the prognosis of treatment of osteoporotic vertebral fracture. Methods 17 patients with the osteoporotic vertebra compression fracture who had been treated both by osteoporotic vertebral pedicle screwinternal ifxation and PMMA bone cement augmentation meanwhile from March 2009 to July 2013 were selected into the research and analyzed by the VAS score, X-ray, vertebral height and Cobb angle. Results The average VAS score decreased from 7.5 points before operation to 1.3 points after operation (P<0.05), including 3 cases of bone cement leakage with VAS score decreased from 6.5 points before treatment to 3.7 after treatment. The vertebral height before treatment was (1.7±0.9)cm and increased to (2.7±0.5)cm after pedicle screw internal ifxation with augmentation by bone cement. And the Cobb angle became smaller after operation than before (P<0.05). Conclusion The amount of bone cement is closely associated with the clinical effect of pedicle screw internal ifxation for the treatment of osteoporotic vertebral fracture.%目的:探讨骨水泥在椎弓根钉内固定治疗骨质疏松性椎体骨折中的应用。方法选取椎弓根钉内固定同时应用骨水泥强化的17例骨质疏松性椎体压缩骨折患者,采用视觉模拟(Visual Analogue Scale,VAS)疼痛评分、X线、椎体高度、检测Cobb角方法进行评定预后效果。结果 VAS疼痛评分术前平均7.5分降至术后平均1.3分,差异有统计学意义(P<0.05),其中骨水泥渗漏3例,评分由术前6.5分降至术后3.7分。椎体高度治疗前(1.7±0.9)cm增加至术后(2.7±0.5)cm,手术后Cobb角较手术前减小,差异有统计学意义(P<0.05)。结论骨水泥参与椎弓根钉内固定治疗骨质疏松性椎体骨折的治疗,效果较好,值得临床推广应用。
