丙泊酚中长链脂肪乳对梗阻性黄疸患者手术时肝功能影响

摘要

Objective To evaluate the effect of medium-and long-chain triglyceride emulsion propofol on liver function in patients with obstructive jaundice undergoing bile duct exploration.Methods Forty patients ASA Ⅱ～Ⅲ with obstructive jaundice undergoing scheduled bile duct exploration were randomly divided into group A and Group B (n=20).Group A was maintained anesthesia with 1％ long-chain triglyceride emulsion propofol injection, Group B with medium-chain triglyceride emulsion propofol injection.Electrocardiogram (ECG),blood pressure, and blood oxygen saturation were monitored continuously throughout the surgery.The indices of liver function were measured before and after operation.Results The serum ALT, AST, TBIL and DBIL levels at the end of operation in both groups were higher than those before operation.The four parameters in group B were lower than thoseo in group A after operation.P ＜ 0.05.Conclusion The medium-chain triglyceride emulsion propofol has less effect on liver function in patients with obstructive jaundice undergoing bile duct exploration.%目的:观察丙泊酚中长链脂肪乳剂对梗阻性黄疸患者行胆管探查术时肝功能影响.方法:择期行开腹胆管探查术患者40例,ASA II～III,随机分为A、B两组.A组采用1%丙泊酚长链脂肪乳注射液行麻醉维持,B组采用1%丙泊酚中/长链脂肪乳注射液行麻醉维持.连续监测心电图、血压、血氧饱和度,于术前和术毕测定肝功能各项指标.结果:两组术毕肝功能血清ALT、AST、TBIL和DBIL均升高;但B组与A组相比,其上述4项指标上升幅度明显低于A组(P＜0.05).结论:丙泊酚中长链脂肪乳剂对梗阻性黄疸患者行胆管探查术时肝功能影响小.
