上海市规范化培训医师工作压力现状调查与对策研究

摘要

A questionnaire survey on standardized training residents' job stress was carried out among 318 participants by random sampling from 4 training hospitals in Shanghai.Results showed that mean score of job stress among training residents was 71.1.The most important job stressors were specialty,workload and time budget.One-way analysis of variances showed that total points of job stress among residents were significantly differences among different types of work experience,highest degrees,training years,training hospitals,income satisfaction and recognition of the necessity of residence training program.Stepwise multiple linear regression analysis revealed that income satisfaction,recognition of the necessity of residence training program,training years and work experience were significant for predicting the standardized training residents' job stress which explained 25.1% of the variance.Accordance to the result,effective countermeasure including improving security system of residents' training program,optimizing the rotation schedule and paying attention to humanistic care were put forward.%以上海市4所住院医师规范化培训基地的规培医师为研究对象,采用完全随机抽样方法,选取318名规培医师进行问卷调查.调查结果显示:规培医师工作压力总平均分为71.1分,工作压力源得分最高的是“专业及工作方面”和“工作量及时间分配”.单因素分析结果显示:工作经历、最高学历、已规培时间、规培基地、收入满意度和对规培必要性的认同度不同的规培医师,其工作压力总分差异具有统计学意义(P<0.05).逐步线性回归方程结果显示:收入满意度、对规培必要性的认同度、已规培时间和工作经历进入回归方程,其中收入满意度越高、认为规培有必要者,工作压力越小;已规培时间越长、有工作经历者,工作压力偏大.最后,从完善规培医师的保障体系,合理安排规培医师的轮转计划,重视对规培医师的人文关怀3个方面,提出了缓解规培医师工作压力的对策.
