分泌性中耳炎患者外周血和中耳积液中IFN-γ和 IL-4的检测及意义

         

摘要

目的:探讨分泌性中耳炎(SOM)患者外周血和中耳积液中干扰素-γ(IFN-γ)和白细胞介素-4(IL-4)的检测及意义。方法入选85例 SOM 患者,设为观察组,根据病程划分为慢性组(病程#3个月)和急性组(病程<3个月),另选择同期我院体检的健康志愿者33例,设为对照组。流式细胞仪检测所有受试者外周血及 SOM 患者中耳积液中 IFN-γ和 IL-4,计算 IFN-γ/IL-4。结果观察组外周血中 IFN-γ、IL-4和 IFN-γ/IL-4均显著高于对照组,差异均有统计学意义(P <0.05);慢性组外周血中 IFN-γ、IL-4和 IFN-γ/IL-4均显著高于急性组,差异均有统计学意义(P <0.05);观察组中耳积液中 IFN-γ显著高于外周血中,差异有统计学意义(P <0.05),外周血和中耳积液中 IL-4和IFN-γ/IL-4比较差异无统计学意义(P >0.05)。结论中耳积液中 IFN-γ还可由中耳腔局部产生,SOM 患者外周血IL-4和 IFN-γ高表达,IFN-γ/IL-4失衡可能参与了 SOM 发病中,并能评估 SOM 病情。%Objective To explore the detection and significance of IFN-γand IL-4 In the peripheral blood and middle ear effusion of secretory otitis media patients.Methods Selected 85 patients with SOM,they were set as observation group,and they were divided into chronic group(# 3 months duration)and acute group(course<3 months)according to the course of the disease,then chose 33 cases healthy volunteers in our hospital at the same time,they were set as con-trol group.Flow cytometry instrument testing all subjects in patients with peripheral blood and SOM IFN-γand IL-4 in the middle ear effusion,computing IFN-γ/IL-4.Results IFN-γ,IL-4 and IFN-γ/IL-4 in the peripheral blood of obser-vation group were significantly higher than that of control group,the difference was statistical significance(P <0.05);IFN-γ,IL-4 and IFN-γ/IL-4 in the peripheral blood of chronic group were significantly higher than that of acute group, the difference was statistical significance(P <0.05);IFN-γin the middle ear effusion of observation group was signifi-cantly higher than in the peripheral blood,the difference was statistically significant(P <0.05);while there were no sta-tistical difference between IL-4 and IFN-γ/IL-4(P >0.05).Conclusion Middle ear effusion of IFN-γ can also be pro-duced by middle ear cavity local,SOM in patients with peripheral blood IL-4 and IFN-γhigh expression of IFN-γ/IL-4 imbalances may be involved in the incidence of SOM,and evaluated the SOM.
