中性粒细胞与淋巴细胞比值结合细胞角蛋白19对老年人舌鳞癌早期诊断意义

Objective:In order to discuss the significance of the application which combning the NLR with CYFRA21-1 together about early diagnosis tongue squamous cell carcinoma.Methods:50 cases with without treatment of tongue squamous cell carcinoma were selected as a study group.Selecting the 50 cases of normal people as control groups.To evaluate the NLR to diagnose the critical value of tongue squamous cell carcinoma by using ROC curves,and combining pplication of critical value with CYFRA21-1 in order to judge that whether it have advantages of improving the susceptibility and specificity of tongue squamous cell carcinoma or not.Results:by using ROC curves to evaluate the NLR to diagnose,the critical value were 2.75.The susceptibility,specificity,Youden index,correct index were 0.79,0.85,0.65,0.72.By combing NLR with CYFRA21-1 to diagnose the tongue squamous cell carcinoma's susceptibility,specificity,apositive predictive value,negative predictive value,Youden index.There were 0.82,0.86,5.85,0.21,0.68.Conclusions:The application of NLR have higher susceptibility,and specificity about tongue squamous cell carcinoma.Combing NLR with CYFRA21-1 can improve the susceptibility and specificity of early diagnosis of tongue squamous cell carcinoma.%目的:探讨应用外周血中性粒细胞与淋巴细胞比值(NLR)联合鳞状细胞癌抗原(CYFRA21-1)对老年人舌鳞癌早期诊断的意义.方法:选取来我院行手术治疗的老年人舌鳞癌患者50例为研究组,50例行健康体检的人为对照组,运用受试者回归曲线(ROC曲线)判定NLR诊断老年人舌鳞癌的临界值,并结合CYFRA21-1诊断分析NLR联合CYFRA21-1对早期诊断老年人舌鳞癌的意义.结果:运用ROC曲线分析NLR诊断老年人舌鳞癌临界值为2.75,灵敏度为0.79,特异度为0.85,Youden指数为0.65,准确度为0.72.NLR与CYFRA21-1联合对老年人舌鳞癌诊断的灵敏度,特异度,阳性似然比,阴性似然比,Youden指数分别为0.82,0.86,5.85,0.21,0.68.结论:选择NLR诊断老年人舌鳞癌的敏感性与特异性都提高,NLR结合CYFRA21-1有助于提高老年人舌鳞癌早期诊断的敏感性与特异性.
