输尿管镜下钬激光碎石术治疗输尿管结石合并狭窄的效果分析

摘要

目的：分析输尿管镜下钬激光碎石术治疗输尿管结石合并狭窄的效果。方法观察组患者采用钬激光碎石术治疗，对照组患者采用碎石机（Dornier1V型）体外碎石。对比分析两组患者一次性碎石成功率、治疗效果、术后并发症、手术时间及住院时间的差异。结果观察组患者治愈率和总有效率分别为81.7%（49/60）和95%（57/60），明显高于对照组[64.3%（38/56）和83.9%（50/56）]，差异具有显著性（P＜0.05）；观察组患者术后3例尿血，2例输尿管穿孔，并发症发生率为8.3%（5/60），低于对照组[25%（15/56）]，差异具有显著性（P＜0.05）；观察组患者平均手术时间为（17.5±3.2）分钟、平均住院时间为（7.2±3.2）天，优于对照组，差异具有显著性（P＜0.05）；复查观察组患者结石排净率为93.3%（56/60），明显高于对照组[80.4%（48/60）]，差异具有显著性（P＜0.05）。结论采用钬激光碎石术治疗输尿管结石合并狭窄不受狭窄处影响，安全高效，实用性强。%Objective Analyze the clinical effect of treating ureteral calculi with ureteral stricture with ureteroscopy holmium laser lithotripsy. Method Study group received ureteroscopy holmium laser lithotripsy. Control group received traditional extracorporeal shock wave lithotripsy. Compared the effect, complication, operation time and in-hospital time of the two groups. Result The cure rate and effective rate of study group were 81.7%and 95%, higher than that of control group. In study group there were 3 cases hematuria, 2 cases ureteral perforation, rate of complication was 8.3%, lower than control group. The operation time of study group was (17.5±3.2) minutes, in-hospital time was (7.2±3.2) days, better than control group. The stone free rate of study group was 93.3%, higher than control group with signiifcant difference in two groups (P<0.05). Conclusion Holmium laser has lots of advantages on treating ureteral calculus combined with stenosis, it can’t affected by narrow place, safe and efifcient and strong practicability.
