红细胞分布宽度、糖化血红蛋白水平与不稳定型心绞痛患者冠状动脉病变程度的关系

摘要

目的 探讨红细胞分布宽度、糖化血红蛋白水平与不稳定型心绞痛患者冠状动脉病变程度的关系.方法 回顾性分析2012年4月至2016年4月于本院诊断为不稳定型心绞痛的75例患者的临床资料,探讨不同冠状动脉病变程度患者红细胞分布宽度(red blood cell distribution width,RDW)和糖化血红蛋白(glycosylated hemoglobin,HbA1c)水平的差异,并采用Spearman相关分析探讨其与冠状动脉病变的相关性.结果 多支病变患者RDW、HbA1c水平均明显高于双支病变和单支病变患者(P<0.05);完全闭塞患者RDW、HbA1c水平均明显高于轻、中、重度闭塞患者(P<0.05).单因素分析提示:冠状动脉病变支数与血清RDW、HbA1c水平、糖尿病患病率均呈正相关(r分别为0.24、0.14、0.15;P<0.05);冠状动脉狭窄程度与血清RDW、HbA1c水平、糖尿病患病率均呈正相关(r分别为0.47、0.64、0.34;P<0.05).多因素分析提示:血清RDW(P=0.01)、HbA1c水平(P=0.001)是冠状动脉病变的独立危险因素.结论 冠状动脉病变支数和狭窄程度与血清RDW、HbA1c水平密切相关.%Objective To study the relationship between red blood cell distribution width (RDW), glycosylated hemoglobin (HbA1c) level and severity of coronary artery lesions in patients with unstable angina pectoris. Method Clinical data of 75 patients with unstable angina pectoris in our hospital from April 2012 to April 2016 were collected to investigate the difference of RDW and HbA1c level in patients with different degrees of coronary artery lesions. Spearman correlation analysis was used to investigate the correlation between RDW, HbA1c level and coronary artery lesions. Result RDW and HbA1c level of multivessel lesion patients were significantly higher than those of single vessel lesion patients and double vessel lesion patients (P < 0.05); RDW and HbA1c level of total occlusion patients were significantly higher than those of mild, moderate and severe stenosis patients (P < 0.05). Single factor analysis showed: the number of coronary artery lesions was positively correlated with serum RDW, HbA1c levels and diabetes morbidity rate (r = 0.24, 0.14, 0.15; P < 0.05); degree of coronary artery stenosis was positively correlated with serum RDW, HbA1c level and diabetes morbidity rate (r = 0.47, 0.64, 0.34; P < 0.05). Multivariate analysis showed: serum RDW (P = 0.01) and HbA1c level (P = 0.001) were independent risk factors for coronary artery lesions. Conclusion The number of coronary artery lesions and the degree of coronary artery stenosis are closely related to serum RDW and HbA1c level.
