脉搏波传导速度与同型半胱氨酸及跨室壁复极离散度的相关性研究

摘要

目的 探讨脉搏波传导速度(PWV)与同型半胱氨酸(Hcy)及跨室壁复极离散度的关系.方法 141例患者,根据全自动动脉硬化仪检查结果分为PWV<1 400 cm/s(正常组)和≥1 400cm/s(动脉硬化组).全部研究对象均空腹检测血糖(FBG)、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、血肌酐(Cr)、尿酸(UA)及Hcy.根据12导联心电图测得Tp-e及QT值,并计算得出TDR值即Tp-e/QTc值.对所有研究对象行超声心动图检查,测得心脏结构中相关内径数值.结果 两组间比较,FBG、TG、HDL-C、LDL-C、Cr、UA的差异无统计学意义(均为P>0.05).TC、血压两组间比较差异有统计学意义.动脉硬化组与正常组相比室间隔厚度差异有统计学意义,左心房内径、舒张期末左室内径、右心房内径、舒张期末右室内径两组差异无统计学意义(均为P >0.05).Pearson相关分析显示,PWV与Hcy呈正相关(r=0.737,P<0.05);PWV与TDR呈正相关(r=0.687,P <0.05).结论 PWV与Hcy及TDR均呈正相关,PWV可作为临床心血管事件的新型预测指标.%Objective To investigate the relationship between pulse wave velocity,plasma homocysteine and transmural dispersion of repolarization.Methods According to the automatic arteriosclerosis instrument examination,141 patients were divided into PWV < 1 400 cm/s(normal group)and ≥ 1 400 cm/s (atherosclerosis group).Fasting blood glucose (FBG),total cholesterol (TC),triglyceride (TG),low density lipoprotein cholesterol (LDL-C),high density lipoprotein cholesterol (HDL-C),serum creatinine (Cr),uric acid(UA),homocysteine(Hcy),blood pressure were measured in all subjects.Tp-e and QT values were measured in 12 lead ECG,and the Tp-e/QTc values were calculated,as TDR.Cardiac structure parameters were measured by echocardiography and recorded.Results There were no significant difference in FBG,LDL-C,HDL-C,TG,Cr and UA between the two groups (all P > 0.05).There were significant differences in TC,blood pressure between the two groups.There were significant differences in the thickness of the ventricular septum between the two groups.There were no significant differences in left atrium size,end diastolic left ventricular diameter,end diastolic right atrial diameter between the two groups.Pearson correlation analysis showed that PWV was positively correlated with Hcy;PWV was positively correlated with TDR.Condusions PWV was positively correlated with Hcy and TDR,and PWV could be used as a new predictor of clinical cardiovascular events.
